Suggest Treatment For Diverticulitis

Posted on Wed, 10 May 2017
Question: I was dx w/diverticulitis on 1/25. I have only been given oral abx & pain rx x 3 rounds since then. After 3rd ER visit on 4/17, I finally directly asked for admission for IV tx, gi rest, etc but the doctor declined saying no because "you are not septic". However, he referred me to see a surgeon for colon resection. I do not understand jumping to surgery when they've not even admitted me for more intensive tx after nearlyb3 months illness. I was sick all of February, and in March it varied between quite bad to tolerable. Then horrible again since last week. Also, the ER doc did say that the CT done on 4/17 showed worsening since prior CT. I have diverticulitis of the sigmoid colon, which is also causing horrible bladder/vaginal area pain And ive lost 35 pounds since this started. Should i just wait for the surgical consult first week of May or should more be done now? Thank you.

Brief Answer:
Respected xxxxxxx, hi no need of emergency direct surgery right now.

Detailed Answer:
Respected xxxxxxxxx, hi sorry to hear your issue.
*I would like to ask you certain questions presently :
- Details of all symptoms , severity , effect on present health
- associated vomiting / fever / appetite status
- type / consistency / frequency of stools
- associated illnesses like High BP, Diabetes or others
- presently you are ambulatory / bed ridden / daily activities status
- reports done with scanning.
- consuming tobacco / alcohol / any abuse substances in any form
- sleep status
- type of your personality / anxiety status.
*Till you revert back with all, I would suggest as :
@ Any Diverticulitis even worsening on CT then also by enlarge doesn`t require
directly emergency surgery without giving adequate conservative hospitalized
treatment to the patient.
@ There are best possible molecules available to control the symptomatology
& underlying pathological process by admitting the patient and giving best
conservative treatment. Once Dr. has given it properly, have to evaluate
for the response of the same & then in only refractory cases Surgery is
hard & fast required as last resort not from beginning.
@ Should strongly recommend to get hospitalised with good Gastroenterologist
for the same to get better faster recovery.
@ Till you manage it :
-- avoid all oily / spicy / nonveg stuffs.
-- avoid smoking / alcohol / any abuse substances if any using.
-- take small amounts of food at definite intervals preferably light diet.
-- consume more liquids - preferably boiled water, coconut water, home made
juices, rice water , butter milk etc. to maintain your hydration.
-- keep your Blood sugar , BP under control.
-- maintain chart of your weight on daily basis.
-- avoid stress / anxiety , have adequate sound sleep of 8 hours.
-- BE POSITIVE for best outcome as with advanced medical sciences there are
all the aspects available for every patient according to patient`s desire.
-- YOGA for 30 min. a day without failure helps in building immunity & fight
any mighty pathology to tremendous levels beyond imaginations.
Hope this clears your query.
Thanks for using Health care magic & reviewing my answer.
Wishing you fast and speedy recovery from the same.
Waiting for reverting with details.
bye Dear take care.

Currently, symptoms are moderate-severe x 5 days. Had been mild during first week of April, but note the symptoms never ceased since 1/25. Pain has been in LLQ but began including bladder/vaginal area 4 days ago. The abdominal/bladder/vaginal pain is nearly constant. Mostly is a strong cramping feeling but with periodic sharp pains at rest and even more sharp pains with movement. A sneeze causes absolutely horrible bladder/vaginal sharp stabbing pain. Pain is never below a 6 on pain scale. Id say mostly 7-9. Doubles me over in pain at times. Have nausea but no vomiting. Feel full/bloated/hard to eat. Often with clear liquids trying to heal this. Have lost 35 pounds. As for temp, currently <99 but has reached 102 in recent weeks. ER doc said wbc count was 13,000 on 4/17. Stools are small amounts and feel like i need to pass more but can't. Usually have bm everyday this past week. Semi-formed stool. No blood visible. Other dx = Inappropriate Sinus Tach, RLS, GERD, Factor V, L arm/hand CRPS post L brachial plexus injury. Ambulatory. Rest in bed often with heating pad on abdomen but not bedridden. Do not do usual daily activities that arent absolutely necessary for self-care. 3 CT since 1/25 but i do not have the reports. I have been told on 4/17 that CT showed worsened diverticulitis and was of sigmoid. Several blood tests. Wbc 13,000 and negative urinalysis that im aware of. No tobacco/alcohol/ither abuse substances. Very difficult to sleep because of the pain, but when i finally am able i sleep maybe 6 hours. Pain is worse laying on back and/or with legs extended. Easier to try to sleep in my recliner with legs closer to body and leaning to my side. Generally, i am stressed. I know that can worsen this. Im trying my bedt to decrease the stress. I dont know how else to try to get hospitalized. Primary care office doesnt admit. ER doc declined (as i noted in my question) and i can't understand why due to the severity of symptoms .

Detailed Answer:
Respected xxxxxxxxx, thanks for immediately reverting with rest details.
I evaluated all thoroughly, everything narrates your need of hospitalisation for definite management of the same ,Still surgery is mandatory at all is not evident from this. Now options are :
* To change the consultatnt.
* To seek for Private hospitalisation with mediclaim coverage.
* Welcome to XXXXXXX
@ Still I would strongly recommend life style modifications are at it`s best, adhere
to it and have Confident from inside by kicking all depressive thoughts till you
get hospitalised & rest of your upcoming life.
