Suggest Treatment For Diarrhea Due To Antibiotics Intake For Primary Complex

Question: Hello, My 26 month old kid was diagnosed by modified Primary complex, and is been given Akurit Z Kid, today is just 2nd day, but baby is crying madly, with signs of pain in stomach and Penis. He was also given antibiotics because of which he has had 8 loose motions since yesterday.. Please suggest something

Antibiotic associated diarrhea

Detailed Answer:
Hi....Thank you for consulting in Health Care Magic. By what you quote, your kid seems to be having antibiotic associated diarrhea. Management goes as here -
1. Review the diagnosis why antibiotics were started apart from Primary Complex treatment in the 1st instance.
2. If its not true or valid indication for antibiotics- stop them.
3. Specific probiotics like saccharomyces boulardii are indicated for antibiotic associated diarrhea. Its available as G-Norm XXXXXXX to be given twice a day for 5 days, mixed in water or milk.
4. Would like to know if there is any Tuberculosis contact with the kid in the past 2 years.
5. Was Mantoux test performed- if so with what strength- 2 TU or 1 TU or 5TU. According to WHO only 1 or 2 is recommended as above that false positives are high.
6. What was the Mantoux reading?
7. You mentioned penis pain- is there any swelling or redness.
8. Can you send the xray over the website.

In december 2013, he had similar cough which lasted for 1.5 months, baby use to keep coughing day and night, so we were afraid this time..

Antibiotic was started because on last Wednesday he was having dry cough, So Mox syrup was started. However this cough worsened a lot and on Monday the chest X -ray was performed.. It is at this point that doctor started looking into possibility of Primary complex..Doc also looked at growth chart, and it is true that he has not gained any weight for past 6 months, rather lost a few (he weighs 10.4 Kilos at this time - 26 months)

I have attached the chest x ray and doc not on this x ray.. We were advised for Monteux test, which we did last Tuesday.. However at that time baby was coughing so badly that even doctor thought that we should start treatment for Primary complex asap and dont wait for Monteax result to come

I however do not know why he prescribed Antibiotic - Azithral
I have also uploaded the present medicines which are mainly as below:

1- Akurit z kid
2- Prospan and Azithrol
3- Syr Enzocert
4- Multivatim syrup
5- Syr Garderal

But now baby is crying badly and he is trying to show us that there is pain in stomach and penis
He is also running evening fever since yesterday ( fever goes upto 103)

Can you please check the X ray and suggest if we are going in right direction or not ?
Hello doc my wife brother and his wife have suffered from Chest TB, His wife left medicines in between so there was a relapse of TB.. She is still taking Medicines.

first contact with kid was when he was 3 months old, the contact was for a week
After that contact with baby was when he was one year old, and then the brother and her wife were in our house when this problem in baby was worsened on Monday

Could be Tuberculosis

Detailed Answer:
Thank you for the trust in me. I assure you that you are going in the right direction. The 4th question in my suggestion has clinched a very important point that there was positive history of contact with TB case. But there are few issues to be addressed regarding this.
Issues -
1. The relative who has stopped the treatment and restarted falls under - defaulter - category.
2. She could be harboring multi-drug resistant TB organisms and this warrants work up for the kid too.
3. It is always better to arrive at a bacteriological diagnosis before starting TB treatment in this era of Multi -drug resistance organisms growing in the community.
4. I suggest you admit the kid and get the following done under your doctor' supervision - CBC/ ESR/ Mantoux in another arm with 1 or 2 TU if you didn't know this strength earlier/ Nasogastric tube aspiration of early morning gastric juice AFB for smear and culture along with sensitivity.
5. If the baby doesn't improve, kid might even require bronchoscopy and bronchoalveolar lavage fluid for smear/ culture and sensitivity and Xpert MTB Rifa.
6. Do not do gamma interferon test - it is a waste.
7. Stop other antibiotics like Advent and Azithromycin.
