Suggest Treatment For Depression

Question: My father aged 56 is suffering from severe depression from last year.He is havingsevere suicidal thoughts.
He is taking Olay-md as he becomes aggressive.nexito 10,inderal 10,zapiz .5.
Kindly recommend whether these are right tablets and what we should do for suicidal thoughts which is increasing day by day.

I can understand your father's situation.
ECT- Electro Convulsive Therapy is the best treatment approach to treat severe depression in patient with suicidal ideas.It is used all over the world for that. Please ask your father's psychiatrist for the same. It is safe procedure with minimum due anesthesia risk.

About his medicines:
Olay(Olanzapine ) is an antipsychotic used to treat aggression of depression as well as suicidal thoughts control.

Nexito(Escitalopram) should be given 10 mg two times a day.

Zapiz(clonazepam) should be given 0.5 mg 3 times a day for short period to control aggression,make patient relax and make patient somewhat sedated which can help to to prevent suicide attempt. It must be given by a relative.

Inderal for anxiety, in his case I don't think it is useful.

To above medicines Mirtazapine or lithum should be added.
Mirtazapine is a good antidepressant which has different mechanism of action than nexito.
Lithium is the only drug with proven anti suicidal effect.

You can discuss with his psychiatrist about above medicines and move forward.

He previously had anxiety problem that is why we are giving inderal medicine,because of anxiety problem and stress he developed depression.
Which medicine has lithium and mirtazapine?
And please do let us know that the medicines which we are giving have the side effects of developing suicidal tendencies.
Can electric shocks be given to do away with suicidal thoughts and depression.

Inderal 10 mg is very low dose. The usual requirement is 10 mg 3 times a day for anxiety. However, Zapiz will work for anxiety better than the inderal in such cases.

If he had initial problem of anxiety, paroxetine(Pexidep) is more useful medicine than nexito.

Lithium is there in Lithosun and Mirtazapine is there in Mirtaz.

No , none of the medicines he is taking can cause suicidal tendency rather they are for treatment.
     
Yes, ECT is the best treatment for suicidal thoughts and severe depression. In that electric shock is not given directly at all as shown in movies. It is a very scientific procedure. First patient is given short general anaesthesia and then micro voltage shock is given for 2-3 seconds. Alternate day generally ECT is given. Approximately 5-6 ECTs are required.

We are giving manicarb two times a day which is 300 mg.does it contain lithium.
What should be given to the patient to eat at this time.we are strictly vegetarian.
Does ECT provides quick relief from depression and there is no relapse after that.
Are there any side effects of ECT.

Yes, manicarb contains lithium. But I think 600 mg is not sufficient dose it should be increased to 900 mg. Please discuss with his treating doctor about the same.

Patient can eat whatever he wishes and digest well. No specific diet can help or worsen the problem.

Yes, ECT can fasten the process of recovery with medicines. No major side effects as such. Headache, body ache or fever may occur after ECT but these are also relived with paracetamol or brufen.
