Question: Hello illustrious doctors, my name is XXXX and i have been experiencing debilitating problems for years now that no doctor or specialist i have been to can figure out. First off i'm unable to work or do pretty much anything except for read endless information about what could be causing my symptoms which have driven me insane. I have been diagnosed with clinical depression and taken every anti-depressant there is with no success. I have also been diagnosed with crohn's disease which im not sure even is crohn's. First off my symptoms are always the same i can function steady at a low level of activity that requires me to only have to watch and observe or read and think to myself about just one thing and anything else that requires me to be able to think and do at the same is impossible including sports, working out, cleaning house, running errands, conversing with people, even just thinking about things in my environment, movies, books, life anything are impossible and the more i push myself the more symptoms i experience, which come on almost instantly and leave me in a state of exhaustion, frustration and what feels like dehydration. Literally when i start any of these activities initially things start out normal. Im focusing and thinking about what im doing and proceeding to do it, but with in a few moments , i start to feel tired, i cant think i get head pains i get abdominal pains chest pains my heart rate increases my mouth feels dry and it just keeps compounding every additional moment until the process completely breaks down in a few minutes and then im stuck back in my mind trying to figure out what is going on. Its really anything that requires me to be active and focused on my environment. This has been going for around 5 years or so and it like being in prison not being able to do anything but trying to figure this out. I have had virtually every test i can think of and they all came up normal. Normal metabolic panel and blood count. Normal Hormone panel( thyroid, adrenal, testosterone, gh,) Normal MRI of the head, normal cts of the chest and abdomen, normal comprehensive vitamin profile(slightly low vitamin d) normal comprehensive stool sample, also a serum osmality and urine osmality test both normal, my bun/creatinine is around 19 steadily. I was wondering if this could be an inability of the kidneys to keep up with the demand for water when activities increase. Ive tried multiple types of antidepressants and none of them improve the symptoms despite increasing my mood and energy level the rapid breakdown in ability is still present. If anybody has any information for me or any theories at all i would be extremely grateful as i am tired of living in this hell which is stuck in my mind. Also the headaches are getting worse as is the abdominal pains.

Detailed Answer:
Dear Sir ,

Thank you for posting your query on HCM.

Well looking into your problem I would like to tell you that in my opinion your symptoms are related to Fibromyalgia / Chronic fatigue syndrome rather than depression . All symptoms are quite consistent with this but before that a test for autoimmune disease should also be undertaken as fatigue what you say is a cardinal feature of diseases like lupus . I would advise you to discuss this with your treating doctor and if he thinks ANA testing can be done to rule them out .

Rest prima facie it looks like Fibromyalgia only.

Take care.
