Suggest Treatment For Depression, Excessive Crying, Suicidal Thoughts And Chronic Pain

Question: Everyday I am severely depressed, I have crying episodes, thoughts of suicide, I suffer chronic pain and a longtime NSAID to treat my neck, back, and lower back pain. I have had a fusion on C6-C7. I have Pendred's Syndrome which I am dizzy and balance issues. I have a cochlear implant in my left ear, and severe hearing loss in my right. I take Oxycodone every 6 hours 5/325 mg, flexeril every 6 hours, and valium twice a day for anxiety attacks, and tinitus on my my cochlear ear side. I have extreme high happiness and quickly go into deep depression. I keep planning to commit suicide when I get my pain meds refilled, 120 pills a month for oxycodone and flexerial. I have told a close friend of mine in XXXXXXX and I am in XXXXXXX TX. I told him how i feel, and he says to tell my doctor. I have an appointment with him this Tuesday, 9/5/2017. At my pain clinic which has a physical rehab for people with knee or hip surgery. I do have a disc protrusions in L5 and S1 and when I answer their questions on their form and the part that asked if I feel depressed, and I have answered it Yes the last three months. They don't even read them or question me about my depression. I am seeing a new orthopedic doctor, because the current one thinks I need physical therapy and I am very active running a XXXXXXX home park. I have two masks one for the people around me have no clue that I am depressed, and when I am home alone I am depressed. When I get my refills, I look at my chihuahua name XXXXXXX she senses my sadness and does not leave my side and always gives me kisses and that moment my thoughts of suicide and leaving her behind stops me from persuing thoughts of suicide. I am in constant pain 24/7 and I was in a bad car accident in September 2012 and the disk protrustions was diagnose then, I had 3 epidurals and the pain is there in a month. I get acute stabbing pain in my thoractic area, neck pain comes and goes, and my lower back pain that goes down my left leg. If I cross my legs when sitting my leg is numbed, It does not matter if I cross my left leg or right leg, it still gets numbed and it takes a while to go away.

Treatment of depression is through a combination of medications and Psychotherapy. Medications used are SSRIs (Like Zoloft). However, Zoloft can interact with both Oxycodone and Flexeril increasing serotonin levels in the body leading to serotonin syndrome. So the SSRIs should be started at the lowest dose and slowly titrated up with close monitoring for development of side effects.

I would recommend talking to your primary care provider would be the best option here, who can refer you to a Psychiatrist.
