Question: severe L4-% spinal stenosis with superimposed cental and left-sided small disc herniation identified by MRI. what to do?

Brief Answer: You are suffering from lumbar disc prolapse. Detailed Answer: Hello XXXX Thanks for writing to us I have gone through your history in detail.You have degenerative disc prolapse of lumbar spine. Degenerative changes occurs in spine due to aging and development of weakness in annulus fibrosis which allows herniation of disc. Herniated disc is causing compression over central spinal canal and neural foramen. Treatment for degenerative lumbar disease includes physiotherapy or surgery. Effectiveness of both modalities are same.So,physiotherpay should be tried first. It causes strengthening of Para spinal muscles and it prevents further changes. In acute cases,ideally physiotherapy should be started after pain subsides . Surgery should be the last option. There are also medicines like capsule pregabalin available for pain relief. Other pain relieving modalities are Ozone ablation,Radiofrequency ablation,percutaneous disectomy etc. Treatment can be planned after consultation with neurosurgeon. You also need estimation of serum calcium,vitamin D level and thyroid profile.If there are any alteration then it should be corrected. You should also modify your life style like don't abruptly bend forward and avoid standing for long time.Don't lift heavy objects.While standing position try to keep your one leg in folded(relax) stage so entire body weight comes to one leg.You can switch body weight to another leg periodically.
