Question: hello dr kakkar ,i hope u are fine and remember me ,we had a long chat about hairloss sometime back.
i have dandruff ,moderate,i would not say severe but moderate my doctor has prescribed me keroglo AD shampoo which i am supposed to use for twice a week .
and on the remaining days of the week ,she suggested me to use keratone hair oil 1 hour before hair wash and then after one hour wash it with kerawash shampoo everyday other than the keraglow day.
1.what are ur thoughts on this ??do u think this is a sound treatment for dandruff ????
i have acne problem again moderate (4-5 pimples at a time ),mostly on the forehead along the hairline and near ear side --is this acne vulgaris or is this acne due to dandruff as i feel this mostly occurs on the hairline and i dnt get acne on the cheeks or the nose or anything only at those places where there is hair
she suggested me a clindamycin gel on the week days and on the weekends(sat sunday) she suggested me to use microbenz 3.5 aqueous solution (i will come on this later)
i have some hyperpigmentaion on the neck and forehead region ,she feels it is a commbination of tanning and acne pigmentation and then acanthosis nigricans
so thats the reason for microbenz usage on sat/sun and clindamycin for acne
what are ur thoughts on the above treatments doctor??AND WHAT ARE GOOD TREATMENTS FOR BOTH HYPERPIGMENTATION AND WHAT IS ACANTHOSIS NIGRICANS???

Hi.

Mild dandruff can be effectively controlled with an antidandruff shampoo. Keraglo- AD is a ketoconazole based shampoo which is quite good. You can adjust the frequency of its use according to your needs e.g you may even use it daily Or else you may use keraglo-AD shampoo every alternate days Or may be twice weekly and a regular shampoo on the remainder of the days of week. I agree with the treatment protocol that your doctor has asked you to follow except that you can be flexible with it according to your needs.
Acne and dandruff frequently co-exist. The basic underlying factor in both these conditions (acne and dandruff) is overactivity of oil glands and microorganisms which feed on this excess oil. Therefore, acne on your forehead is part of the same underlying condition.
You can use either clindamycin Or benzoyl peroxide (Microbenz) for mild to moderate acne on forehead. I suggest you to apply either clindamycin Or benzoyl peroxide during the day.
At night I suggest you to use a topical tretinoin preparation e.g Tretin 0.025% Or Airol 0.05% cream, over the affected area on forehead i.e for both acne and pigmentation. Tretinoin is exfoliating and comedolytic and therefore would help with both acne as well as pigmentation.
Tretinoin is a prescription product and I suggest that you use it in agreement with your doctor.
Acanthosis nigricans presents as pigmentation and thickening of skin which therefore appears velvetty, rough in texture. This condition is commonly seen on neck, specially in those individuals who are overweight and is associated with insulin resistance. Acanthosis can also present on other sites like elbows, forehead, armpits, groin etc. Acanthosis nigricans also improves with topical tretinoin.

thank you dr kakkar ,but i have few doubts
1. can u suggest me some mild shampoo for daily cleansing other than the days where i apply keraglo ad .the kerawash shampoo whioch my derm recommended is a bit harsh and i dnt have time to oil my hair and wait for one hour and then shampoo.can u suggest me some mild cleansing shampoo for daily use which can be used in conjunction with the keraglo ad shampoo????

2..does microbenz have any exfoliating properties ,will it help in hyperpigmentation ????intitally my derm suggested me to use epiduo gel then she changed it as the climate is a bit cold here and she felt it might make my skin dry ,so she suggested me to try out the micro benz cream .what are ur thoughts on epiduo and micro benz for hyperpigmentation ,which will be better ??

3..i have vit d def ,my level is around 5ng/dl .i am already taking 60000 iu of calcirol per week and after two months of it ,i am now taking 60000 uiu once a month as suggested by my doctor .i keep my legs alone out in the morn sun for 10 min ??is this necessary or do u think supplements alone would do the job ??
if necessary can u tell me for how much time should i keep my legs in the sun and at what time of the day would be preferable in order to minimize the sun damage (i only keep the legs in the balcony where there is sun exposure and apply sunscreen to protect my face )


4....my doctor said that ,if creams do not do much in my case of hyperpigmentation she would do a chemical peel ?what are ur expereinces with chemiocal peel ??have u had any good results with it and how much can a person expect a reduction in hyperpigmentation(i mean % reduction of hyperpigmentation from baseline )?? would u also do the same for hyperpigmenatation??

5..i am already taking follihair for my hair health ,it lists soya protein and grape seed extreact as its ingridients .i have moderate gynecomastia -combination of both the fat and the gland ,do u think that taking follihair would aggrevate gyno???

6...just like follihair are there any supplements to be taken for skin health ??if yes can u suggest me some...please mention the name

7...u mentioned insulin resistance and weight gain ,though i am not overweight but i have added upon nearly 10 kgs of weight over the past 12 months ,i will reduce that
are there any diet/lifestyle modification which u would reccomend like not eating refined sugar/processed food and eating only a low glycemic diet etcetc

Hi.

1. Triclenz shampoo.
2. Microobenz has keratolytic/ exfoliating properties which helps acne but it is not a treatment for hyperpigmentation.
3. Since you are taking supplements that is sufficient. However, just a few minutes of early morning sunlight is enough for synthesis of vitamin D in skin.
4. Chemical peels like glycolic peel, retinol peel are good for hyperpigmentation and can be combined with topical treatments. Usually a series of sessions i.e 6 or more sessions is required to have noticeable improvements. The number of sessions would vary from individual to individual and depend on the degree of hyperpigmentation.
5. Follihair would not affect gynecomastia.
6. You can take an antioxidant regularly e.g Cap. Antoxid- HC once daily. Follihair is for hair.
7. Take 4-5 small meals throughout the day rather than 2-3 big meals. Take less of refined sugar and carbohydrates. Items like pizzas, pastas, burger contain maida which is refined flour and is not good for body. You should avoid these. Add more of fruits, salads to your diet.
In addition I suggest aerobic exercises like brisk walking, jogging or treadmill for 30 minutes daily.

1.doctor ,i tried and brought the triclenz,i am sorry but i feel even that is harsh on my hair ,can u suugest me some other mild shampoo which is only suitable for cleansing and can be used along with the keraglo ad shampoo so that i can try that ?

2.tab antioxidant --hc is not available inm my area ,can u suggest an alternative .
is it safe to take along with follihair in the afternoon and does it cause any side effects ???

3.i will follow everything u say ,personally how much improvents u see when u do chemical peel after 6 sessions ,the problem is i am fed up with hyperpigmentation and will definitely like to improve that ,and i am not sure about the results which people get ,that why i am asking u again ,do u see good results with it doctor(like 30 % reduction in pigmentation etc)and are chemical peel results permanent as long as i maintain them by not going out in the sun ,using sunscreen and healthy eating/no sugar/no stress /no processed foods etc )???

4.what is the difference between nizoral and keraglo ad ??is nizoral better ??should i use nizoral ????

You ay try if you can find 'Mildy shampoo' Or 'sebamed's revitalising shampoo'.

An alternative to antoxid is Derantox capsules.

With Chemical peels, one can expect a good degree of improvement in pigmentation. The percentage of improvement varies depending on how dark or deep the initial pigmentation is, and it would have a bearing on the number of sessions of chemical peeling. My experience is that on an average one can expect 30- 50% improvement after 4-6 peeling sessions along with regular use of sunscreen and a topical pigment reducing cream at night e.g golite cream (a kojic acid based cream)

Nizoral and keraglo are both antodandruff shampoo. Both are ketoconazole based. Keraglo has icthyol pale in it which is antiinflammtory and therefore of additional benefit in dandruff. Keraglo-AD does not dry hair as much as nizoral does because it contains conditioner, so i prefer keraglo-AD.

thank you doctor ,i have decided to do a chemical peel and even talked wuth my doctor about it ,i am going to undergo glycolic peel for 6 sessions next year feb as discussed with my doctor ,till that time i am supposed to use epiduo cream .
my last question is ,
my hyperpigmentation is a combination of acanthosis ,sun damage from childhood and acne pigmentation ,so as i long i maintain these parameters with suncreen ,weight control,acne control ,the results of the 6 chemnical peels would be permanent right??????
