Question: My mother is having cracked heels fissure and is very painful as it spread to all bottom of the leg, consulted various doctors and followed the prescription but it's give short term relief and in some time it start over again.

Hello and welcome to healthcaremagic

I am Dr. Kakkar. I have gone through your query and I have also looked at the picture.

She has Hyperkeratotic eczema (HK Eczema), as can be seen in the pictures. The skin is dry, scaly, fissured and raw at places. There must be associated itching. The fissures must be painful.

I would suggest an intensive moisturising cream to be applied twice daily e.g Xerina cream (contains glycolic acid + cetylated fatty esters and is a specially formulated preparation for severely dry, fissured and cracked skin) Or Amylac Cream (contains Ammonium lactate 12 % which is also an excellent moisturiser for dry, scaly, cracked skin) twice daily. The moisturiser has to be continued forever even after the fissures heal and eczema resolves

After the moisturise, she may apply an ointment containing a potent topical steroid + salicylic acid as ingredients (Dipsalic-f ointment) twice daily over the patches for 4-6 weeks till the fissures heal and eczema gets better.

An OTC oral antihistamine e,g cetrizine would provide her symptomatic relief from itching
