Suggest Treatment For Cough And Wheezing

Question: HI,
My 10 months onld boy has cough very often, when he was 4 months old he got severe cough when i took him to my pediatrician she said he is having wheezing 2 days twice i gave asthalin through nebulizer. when he was 8 months old we went to austria whereas we went for upper austria for a week vacation, there due to cold we kept 2 room heaters that dried his respiratory he started to throw some scary sound (dried sound) after 2 days we came to XXXXXXX from that time the cold reduces sometimes and it comes again. 2 weeks ago also he had severe wheeze doctor said Asthalin nebulisation every 2 hours for 2 days, then for 2 days every 6 hours then every 8 hrs for 4 days then 7 days of budecort. after that one midnihgt he cried very badly took him to child hospital in the midnight, there they said though he has wheeze sound he is active, so give asthalin whenever required. now today again he is coughing. I am more worried. I am a working mom dont know what to do. Kindly advice me. People says Asthalin nebulization and syrup given for Asthma patients, I am more worried when I hear this. kindly advice is Asthalin given for Asthma patients?

Hi...I understand your concern. First of all I want to alleviate your fears about labeling your kid as asthmatic. According to the latest XXXXXXX guidelines, a kid less than 5 years should not be labeled as asthmatic, rather it could be either a multi triggered wheeze or a viral episodic wheeze.

I have a few questions for you -
Questions:
1. How many days per month does he cough or feel breathless?
2. How many nights per month does his sleep get disturbed due to above symptoms?
3. Does he feel breathless when he is actively playing?
4. Are the symptoms when there are seasonal changes?
5. Is there any family history of asthma or any other sort of allergies like skin allergy or eczema etc.?
6. Is the cough always associated with fever?

Based on your answers to the above questions, I would be able to decide on what is going on in your 10 month old kid and what is the best management for the above problem.

1. from September5th - till today he got this breathless and cough 3 times (it started on 5th when we took him to Upper Austria(Halstatt) where it was very cold so kept 2 room heaters whereas he got driedness and making some sound and severe cough.
After coming to XXXXXXX after 4 days he started to cough more took him to doctor, my Paed suggested ashthalin syrup for 5 days with Zinc powder.
Follow-up she said no chest cold its fine.
But after 10 days again he got cough with wheeze took him to Paed she suggested the nebulization as I mentioned in my first post.
after giving asthalin (nebulization) for 3 days he was fine. End of 3 days he was crying badly took him to different hospital there they gave ear drops he was fine, after 2 days we took him again to the same doctor for followup, he said whenever he has wheeze you give nebulisation. and also he said he is active no chest cold but slight wheeze is there but thats alright. he told us to give montair granules for 2 months.

he was fine after than, but last night he was crying very often, and this morning he was coughing while sleeping it seems, but now he is fine (he is in daycare).

2. I can say this month may be 3-4 nights he spent by crying, in that 1 night severe, 1 night due to ear pain.

(Please note: This is the second time he got wheeze in 10 months, but whenever he rolls down he was having little sound kind of fast breath.)

3. when he is playing no problem only during evenings he has this issue i beleive (most of the times). when he was rolling down during 4 months he was little bit fast breath, that doctor told because he is a chubby boy.

4. yes it occured first when i took him to other city, and now also when the climate change he got (I can say most of the times it happened due to climate change)

5.my husband's father has a asthma (wheezing) problem. My husband also has cold issue - very often he cathes cold.

6. There is no fever only once he had fever when he had cold and cough

Multi triggered wheezing

Detailed Answer:
Hi...thank you for the detailed answer and by the history you have provided me, your son's problem fits into text book description of multi triggered wheeze (MTW). As the name suggests, it is a wheezing problem due to various triggers like cold weather, climatic changes, cockroaches at home, dust mites, dust, cobwebs, paint or petrol smell etc. But it is cannot be attributed as asthma at such an young age.
But whatever is the name, the symptom is wheezing and cough and breathlessness. The best drug for this is Salbutamol or Levosalbutamol. These drugs are best administered by inhalational route than oral route.
I suggest these medications for your kid and you consult a paediatric pulmonologist in your place and get hold of the technique of how to administer these drugs as I am going to advice you to use inhalers.

Suggested medicines - Budecort 100 2 puffs 12th hourly for 8 weeks and Levolin Metered dose inhaler 2 puffs every 4-6th hourly only if there is cough or wheeze. Both these drugs should be administered by a spacer and this is what I was asking you to learn - the technique of giving a metered dose inhaler with a spacer. I also suggest you to use Montelukast for 8 weeks.
