Suggest Treatment For Cough And Blood In Mucus

Question: Hi ,

I am having cough for past 30 days . I used antibiotics initially . Now I am using Benadryl .

I observed little blood in Flem / Mucus for the past two days . That was Flem coming in dark red color and sometimes little of blood coming while coughing . I am little worried and anxious . Could you please help me to know what might be the cause , is it serious ? I dont have any other serious symptoms as of now.

Thanks for your query.

Regarding the symptoms as mentioned in your post, kindly let us know about the following:
1. Is the cough associated with breathlessness and chest pain?
2. Any diurnal variation? i.e. symptoms are more during any particular time of the day?
3. For how many days did you take the antibiotics?
4. Any tests/ laboratory investigations carried out? If yes, then please attach the concerned reports along with your post.
5. Any past history or family history of Asthma, Pulmonary Tuberculosis, Hypertension, Diabetes, Thyroid disorder, etc?
6. History of exposure to any chemical or smoke at home or working place?
7. Habit of smoking or alcohol consumption?
8. Any other associated symptoms like fever, loss of weight and appetite, etc.?

A knowledge of the above mentioned facts will be helpful to make a better diagnosis and formulate an appropriate management plan.

Till then, symptomatic relief can be obtained with:
1. Warm salt water gargle and steam inhalation.
2. Antihistamine preparations like Cetrizine or Allegra.
3. Continue the syrup to control cough.
4. Avoid taking cold food & drinks, maintain adequate hydration & take soft, non-spicy foods.

Hi Doctor ,

Please find the answers below .

1. Is the cough associated with breathlessness and chest pain?
[ No ]
2. Any diurnal variation? i.e. symptoms are more during any particular time of the day?
[Generally in the morning the cough is less and during the evening it is little high and in the night it is fine . I dont observe any cough in the night or no disturbances for sleep ]
3. For how many days did you take the antibiotics?
[ 5 Days and daily 3 tablets of amoxilin ]
4. Any tests/ laboratory investigations carried out? If yes, then please attach the concerned reports along with your post.
[No Tests carried out as of now ]. Just the normal GP VisitS.
5. Any past history or family history of Asthma, Pulmonary Tuberculosis, Hypertension, Diabetes, Thyroid disorder, etc?
[No]
6. History of exposure to any chemical or smoke at home or working place?
[No]
7. Habit of smoking or alcohol consumption?
[No]
8. Any other associated symptoms like fever, loss of weight and appetite, etc.?
[No no other symptoms found ]

I wanted to mention the Phlem with Blood color I observed only when in the morning after wakeup and first few Phlems are with blood and yellowish in color ].After that during the day the Phlem is coming out but no blood in it .

The cough was iritating from past 30 days it is dry and itchy in the throat .

Kindly guide me .

I will do the rest of the procedures to minimize the effect.

Thanks for your follow up query.

Considering the details of the case history as mentioned in your post, the symptoms can be suggestive of either allergic Pharyngitis or recurrent Upper Respiratory Tract Infection (URTI). Presence of blood in the phlegm is due to minor trauma to the respiratory passages due to repeated irritation and bouts of coughing.

A. Allergic Pharyngitis: seems to be the probable cause. There was a superimposed infection previously, which has subsided due to intake of antibiotics (Amoxicillin). However, the allergic reaction will keep on recurring on exposure to cold, dryness of throat, dust or any other allergen.

Management will consist of:
1. Antihistamine preparation like Cetrizine or Allegra. You can use it during an acute attack.
2. Montelukast preparations- used as a maintenance therapy.
3. Anti-tussive preparations for symptomatic relief from cough.
4. Symptoms aggravating in the presence of a superimposed respiratory tract infection needs to be managed with antibiotics.
5. Also make sure that you are well protected from exposure to cold, dust & other allergens.
6. Maintain adequate hydration & avoiding cold food and drinks. Steam inhalation therapy will also be helpful.
7. Adequate voice rest & deep breathing exercise.

B. Recurrent Upper Respiratory Tract Infection (URTI): If the symptom causing agent is not susceptible to the antibiotics prescribed, or if there is any hidden focus of infection in the nose/ throat/ ear, then also the symptoms will recur.
Under such circumstances, a proper clinical examination of the Ear, Nose and Throat needs to be performed to detect any hidden focus of infection and a course of antibiotic to be prescribed based on culture & sensitivity reports of a throat swab.

So, kindly consult your treating Doctor and discuss regarding the same.
