Suggest Treatment For Continuous Negative Thoughts And Fear

Question: Hi I'm very much afraid about my life. Always negative thoughts comes in my mind.
Death thinking comes in my mind. whenever i get ill or get pin in chest then death thought comes in my mind.
Afraid about my family, my 6 months and 5 yr old son.What will happen to them if something happens to me.
Yesterday i got difficulty in taking breath and thought came if i sleep then whether in morning i wakeup or not.
I need psychiatric help.
very much afraid in night.
Due to fear i avoid to travel in airplane.

I can understand your concern. The symptoms you have mentioned like fear about health condition, fear or anticipatory anxiety if you will wake up or not, fear of death, fear about future of your children etc suggest severe anxiety most likely generalised anxiety. Breathing difficulty you had yesterday was also due to acute anxiety attack.

In Generalised Anxiety Disorder persistent anxiety in day to day routine is seen. A lot of individuals suffer from this condition and fortunately this is treatable. With proper medicines and behaviour therapy symptoms of most individuals improve.

Commonly SSRIs are used for anxiety. Fluoxetine, Paroxetine, Sertraline etc are used. These drugs are safe drugs and free of serious side effects. In most individuals response is seen in about 4-6 weeks of starting treatment. For control of acute symptoms like you had yesterday drugs like Clonazepam or Lorazepam etc can be used. These drugs can calm you in acute anxiety like situations. Please contact a good psychiatrist for proper evaluation and prescription of these drugs.

Behaviour therapy along with relaxation exercise is other modality of treatment. Breathing exercise, yoga, progressive muscle relaxation exercises are used in behaviour therapy for control of anxiety. For best results combination treatment is used.

What is behaviour therapy?
My problem is anxiety only or some fear also.
I feared of crowded place also. My fear is travelling in airplane since last year when i was in plane with my wife and son and plane stucked in turbulence due to which i got very depressed and couldnt eat anything in whole journey.
Is this also anxiety. or something else.
I already consult a psychiatrist in oct 2015 and he told me problem of serotonin.
He prescribed me "censpram 10mg, joynap md0.5, cap luxury, and tab Etilaam md0.5 mg sos in anxiety.
I took medicines for 2 months and stopped.

My other problem is also whenever i saw or heard some death news of anyone, i co-relate it with my life and my mind goes deeply in it.

My life is very disturbed and couldnt concentrate on my work.

Hello again

Behaviour therapy is a modality of psychological treatment in which relaxation exercises are taught to the patient and then patient is advised to relax himself in anxiety provoking situations by relaxation exercises. A good psychologist can help you in Behaviour Therapy. Like in your case fear of crowded places can be reduced by systematic desensitisation or progressively increasing anxiety provoking stimuli are given and relaxation is done. The symptoms in your case are occurring due to anxiety only. Fear of crowded places originates due to agoraphobia which is part of anxiety.

Censpram is Escitalopram and the drug is a good SSRI drug. SSRIs help to improve serotonergic neurotransmission and thus reduce anxiety and depression. The drug if taken regularly helps to improve anxiety. Joynap and Etilaam both are benzodiazepines and should be reserved for severe anxiety provoking situations like while in crowded places or during flight. These drugs can reduce acute anxiety symptoms quickly.

If possible visit your doctor again and talk to him. The drugs will definitely help you to come out of your symptoms. No need to worry.
