Hi madam.Thanks for your query.

Treatment of Chronic urticaria is a difficult goal to achieve in clinic practise.However I would like to suggest and help you to maximum possible extent before which I would like to know some more information about your current condition-
What are symptoms that you have been experiencing?Do they come repeatedly?
If yes how frequent?How long do they stay?
When do they increase/aggravate?Has any other probable cause been identified by your physician?
What are medications that you have been taking all this time?Can you upload the scan of prescription/reports using the report tab on your screen?
Have you been taking them as per your dermatologist's suggestion?

- Swollen itchy patches any where in body

Do they come repeatedly? - yes

If yes how frequent? How long do they stay? - currently thrice a week n stays few minutes

When do they increase/aggravate? - never noted
Has any other probable cause been identified by your physician? - no

What are medications that you have been taking all this time? - anti-allergens (Avil, cetirizine etc.)

Can you upload the scan of prescription/reports using the report tab on your screen? - at present do not have any

Have you been taking them as per your dermatologist's suggestion? - yes very much
Even have done homeopathic for 2 years and thought its cured, but came again after 1 year.

I would recommend medication depending on the response

Detailed Answer:
Hi madam.Thanks for the revert.

I am glad that you have answered all my questions in very structured manner and I deeply appreciate your patience.

I were to prescribe medications as mentioned below depending on the response and urticaria manifests-
a) Antihistaminics (cetrizine, loratidine) to start with
b)Titre the dose to high dose antihistamincs if the symptoms donot subside or add a combination of anti-allergy medications like montelukast.

I would recommend these medications upto 3 months depending on the response.

A first-generation antihistamine such as chlorpheniramine may be considered if you are planning to conceive.

In case of repetitive nature of symptoms then I were to prefer choose colchicine or systemic steriods(prednisolone).

As a last resort based on further evaluation for autoimmunity I were to choose immunonodulators such as Cyclosporine.

Additionally few suggestions below might help you-

Avoidance of mental stress, overtiredness, alcohol, nonsteroidal anti-inflammatory drugs (NSAIDs), and tight-fitting garments. Psychological stress can trigger or increase itching.

Nocturnal itching reduces by lukewarm bathing and keeping the ambient temperature of the bedroom cool.

Application of lotions with menthol and phenol provide relief of itching.
