Suggest Treatment For Chronic Cough

Question: Hello,
I am suffering from chronic cough for around 4 weeks. I have been observing this pattern once every year in the winter. Last year I went to a Pulmonologist and have done my sputum tests, CBP and Chest X-ray done and everything was normal. But I was coughing a lot. The clinical diagnosis was reported as Post Nasal Drip and I was prescribed with Foracort Revolizer Caps, Montelokaust, Medrol 32MG (for 4 days), Azythromycin course, Duonase nasal decongestant and a cought syrup I don't remember. I was better in 5 days. I could witness the improvement the very first day of taking medicines.

Now the same episode repeated this year. As I moved out of that place I had no access to that doctor. I tried all conservative techniques like salt water gargle, turmeric water, steam, garlic etc. One night the cough was extreme and so I have taken one Medrol 16MG, one Foracart revolizer inhale. I was better that day. But as Medrol is a corticosteroid I was bit apprehensive and went to another pulmonologist available near by. The doctor said it was simple Postnasal drip and it wont require any of those drugs except for the Livocitrezen, One course of Azithromycin, Saline nasal spray and a steroid nasal spray. I have repeated the tests this year too. Everything was normal and had slight ESR values like 12 in the first hour and 26 in the second hour. I felt better for the three days of taking Azithromycin. And the cough got aggravated there after. I am still suffering from cough and it is getting prolonged. I am frustrated as I am unable to do anything. I am not able to do any exercise at all. It is causing me cough. This time the cough episode is taking longer. I am very much concerned. One friend suggested that a peg of brandy would make me better. I tried one peg of brandy with plain water and I felt better that night. But the cough persists from the next day. Especially during mid nights. I am having sleepless nights. Please help me get through this. Thanks a lot in advance.

Thank you for your query. I can understand your concerns.
It seems that you are having recurrent/seasonal allergic rhinitis with post-nasal drip.
Nasonex nasal spray (mometasone), monteleucast (10 mg), levocetrizine(5mg) orally for 3 months at least, along with a short course of steroid (deflazacort 6 mg) orally will help.You also need to take a course of broad-spectrum antibiotic(amoxycillin- clauvulinic acid combination) for about 05 days as it seems that lower respiratory tract involvement is not unlikely in view of persistent cough.

The aforementioned medicines are prescription only medicines which I am sure your local doctor will know about it.

Hello Dr. Biswas!

Thanks for your reply. I have certain concerns on it. Before this cough attack I was working out regularly and was on proper diet and was loosing extra fat. I was healthy and happy with all that. Now I am unable to do any exercise. I have once tried and it aggravated my cough.

As per your suggestion of three month course of medication, can/will i be able to do exercises? Or should i stop that?

Hi,
Seasonal allergic rhinitis is temporary and intermittent and is likely to subside as it happened in previous year. Avoid very early morning exercise as cold air may trigger the symptoms.Exercise to the limit of tolerance.
