Suggest Treatment For Chills, Shivering And Fever In A Child

Question: HI SIR/MAM MY NAME IS XXXX AND MY SON'S NAME IS XXXX AND HE IS 3 YEARS OLD.. HIS WEIGHT IS 14 KG... YESTERDAY AT 3.30 AM HE HAD CHILLS AND HIS BODY WAS SHIVERING. AND HE GOT 103 DEGREE CELSIUS FEVER.. THEN I GAVE HIM PARACETAMOL BUT FEVER DIDN'T GO.. AND SOME CHILLS WERE ALSO THERE.. AT 8 AM I WENT TO DOCTOR. HE CHECKED HIM AND HE TOLD ME IT MAY BE VIRAL BUT MALARIA POSSIBILY IS ALSO THERE.. HE TOLD US CBC AND MALARIA TEST.. AND HE ALSO PRECCRIBED PARACETAMOL 250 MG 4 ML SOS AND cefpodoxime CEFPODOXIME PROXETIL SYRUP 100 MG 3.5 ML TWICE A DAY.. AND ARTEMETHER WITH LUMEFANTRINE SYRUP 4 ML TWICE A DAY... MY SON'S WEIGHT IS 14 KG... AT NOON I GOT CBC AND MALARIA TEST DONE.. AND MALARIA REPORT IS NEGATIVE... PLATLETS ARE OK HB IS ALSO OK.. BUT LAB DOCTOR TOLD ME IT'S INFECTION AND TLC IS TOO HIGH... AND OTHER THINGS ARE ALSO SHOWING INFECTION BUT I AM NOT GETTING AND LAB DOCTOR ALSO DIDN'T TELL ME.. HE TOLD ASK YOUR DOCTOR... BUT DOCTOR IS NOT AVAILABLE NOW.. AND HE WILL MEET TOMORROW IN THE MORNING... BUT I CAN'T WAIL TILL MORNING COZ I AM WORRIED ABOUT MT SON.. SO I WANT OPINION FROM YOU ... SO PLZ TELL ME AFTER SEEING REPORT IS IT ANY SERIOUS INFECTION OR IT'S OK... N PLZ TELL ME ALSO SHOULD I CONTINUE MALARIA SYRUP OR STOP IT...

ONE MORE THING I WANT TO TELL YOU THAT MY SON GETS VIRAL INFECTION OFTEN... SOMETIMES IN A MONTH AND SOMETIMES AFTER 1 1/2 OR TWO MONTHS... BEFORE THREE OR FOUR MONTHS HE GOT URINE INFECTION ALSO.. SO PLZ GIVE MY ANSWERS AFTER SEEING REPORT...

Looks like viral illness - stop antibiotic and antimalarials too

Detailed Answer:
Hi....by what you say, I feel that your son is going through a viral illness.

PLEASE STOP ANTI MALARIALS AND ANTIBIOTIC (CEFOPODOXIME) ALSO.

I will explain you how a viral illness behaves so that you can be more confident about your son.

Fever of few days without any localizing signs could as well a viral illness. Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day.

If the kid is active and playing around when there is no fever, it is probably viral illness and it doesn't require antibiotics at all.

Once viral fever comes it will there for 4-7 days. So do not worry about duration if the kid is active.

Paracetamol can be given in the dose of 15mg/kg/dose (maximum ceiling dose of 500mg) every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.

I am not commenting in the reports as I dis not get any uploaded documents please

hello sir i have uploaded report.. before also i did but i don't know what happened n why it was not uploaded :-( ... and plz tellme now after seeing it tlc is 18300 is dangerous n what about other things... he has cough also n he vomitted than phelgum came out.. n he is not taking any food only some water he is taking... can i give ondenstrone for vomiting n what should be the dose.... thanks sir

Nothing to worry - do a urine routine test

Hi.... I have just seen your son's report. It is not dangerous, do not worry. But at the same time as there is a history of urinary tract infection, I suggest you to do a urine routine examination and get back to me with the report.

I will guide you further what to do. Meanwhile please continue only Paracetamol as I have suggested.

Hi....You can give ondansetron for vomiting. The dose is 5ml thrice a day 30 min before feeds.

But this is a prescription drug and you will need a doctor's prescription for it.

The only attachment I have here is the CBC report. I have already given my opinion on it. After seeing that only I have asked you to get back to me with urine routine report please
