Suggest Treatment For Chills, Loose Stool And Nausea

Question: Hello Dr.,

My daughter 6years and 6 months is having fever since 14th April. Its started with 100 F(armpit) and from yesterday its 102.5 F with Chills. Doctor has suggested its viral however started Antibiotics clamp kid forte 7.5 ML from today and asked to continue with crocin and meftal P for fever. Today she vomited also and had a loose stool and was having Nausea since morning. He has now also added Lanzol jr. Twice a day. So far he hasn't advised any tests. Kindly advise if the treatment is on right track and what might be causing this for 5 days now.

I have gone carefully through the details and understand your concern. Most often such symptoms are due to viral infections and antibiotics like Clamp Kid Forte are ineffective in such cases. It is a very bad practice to give antibiotics without a confirmation of raised leucocyte count. However since you have started the antibiotics, please complete the course. Please be patient, it should spontaneously resolve in a few days. Do give steam inhalation to your child. You need to give paracetamol (Crocin) to bring down the temperature. If paracetamol is not effective in bringing down the temperature, you need to use Meftal-P (mefenamic acid) which is more effective. However, I do not think that they need to be used simultaneously. Also use tepid sponging. Giving some antiallergic like cetrizine will take care of allergic symptoms

The passage of loose stools is likely to be due to antibiotic associated diarrhea as the normal beneficial intestinal bacteria are also eliminated by the antibiotics. Giving curd or yogurt should help as it contains beneficial bacteria. Give some antiemetic like ondansetron for the vomiting.

Overall the treatment is on the right track. If the fever lasts for more than a week, blood tests to rule out enteric fever would be advisable.

Hi Dr.
Thanks for your consultation. Doctor got CBC, CRP, and Widal test done. WBC is high 17.6 and neutrophiles 74%, CRP is 24 mg/dl, and Widal test has O titers of 1:160.

Doctor suggested that its mild typhoid and asked to continue the Antibiotics.

My daughter was vaccinated 6-8 months back for typhoid so widal test can be false positive? The only typhoid related symptom seems to be fever increasing every day like two days back peak was 101, then day before yesterday it was 102.5, and yesterday it went 104.1. She feels cold and mild chills at the onset of fever.

Could you pls advise if my daughter has Typhoid? Also, not sure why doctor didn't do Malaria test. Does it take long for it to come in blood tests? Do you advise any tests?

High temperatures are common in typhoid.

Thanks for writing back. The raised WBC count as well as CRP indicate the possibility of a bacterial infection. The previous history of typhoid vaccination may have interfered with the Widal test results and hence no conclusions should be drawn. However, there are antibiotics like cephalosporins and ofloxacin that should take care of typhoid as well as other likely bacterial infections. You should talk to your doctor about modification of the antibiotic regimen In order to conclude definitively on whether your child has typhoid, you need to test for typhidot IgM. Unless you think that your child has chills and rigors, I do not feel that malaria test is required. I would also insist on getting a urine routine microscopy and culture done.
