Question: I am tired or sleepy all day. Housework can be done but when I sit down to read I often fall asleep. I have been told I suffer three heart-related conditions.
1 cardiac myopathy
2. Atrial Fibrillation
3. leaking mitral valve XXXXXXX XXXXXXX YYYY@YYYY

Hello,

fatigue and sleepiness may have various causes. I'll mention some.

- drugs: some of them may cause fatigue like the beta blockers. Others may cause sleepiness like alpha-agonists (clonidin). Benadryl may cause fatigue and sleepiness.
- sleep apnea syndrome: too many pauses in breathing during sleep may cause sleepiness during the day and fatigue. It also causes various heart problems like pulmonary hypertension.
- anemia: low hematocrit can be related to both symptoms.
- electrolyte disorders
- hypothyroidism
- infections
etc

In cases like yours I usually start with some basic laboratory tests like a complete blood count, electrolytes, urea, creatinine, TSH, urinalysis and liver function tests. Clinical examination may reveal findings, which may need further investigation. Evaluation for sleep apnea is warranted as well. If you sleep alone, it's difficult to know if your breathing pauses during the night. An ENT and pulmonologist should examine you before proceeding into a sleep study, which is useful for confirming sleep apnea and checking the response to CPAP masks (used to treat apnea).

You heart problems do not seem like enough cause for your symptoms.
