Suggest Treatment For Burning Sensation In The Stomach

Question: Hi Doctor,
I've been having burning sensation right from my stomach to the throat for past 5-6 months and motion is also soft and I feel burning sensation when I pass the stool. Whenever I burp, I can feel that the food (esp., chutney, Veg/Non-veg curry etc.,) that I'd have eaten, touches up to my throat in the form of liquid which is acidic in nature. Some of the foods that irritate my stomach are pizza (which I rarely eat), any masala curries, and if I eat curd or by chance if I eat more due to hunger, the regurgitation is more and I purposefully vomit and empty out just to feel ease in my stomach and throat. I also noticed that if I don’t consume some sort of food at least once in 2-3 hours, I feel terribly hungry(My food habits: 10AM – Breakfast, 1PM-Lunch, 7.30PM-Dinner) In context of this, I had been to Gastroenterologist and he suggested ph metry, which I've undergone. Pls find my history of reports to the recent ph metry report attached. I'm planning to meet the referring doctor tomorrow. He indicated that 7yr GERD problem is not good sign despite being on medication, and that if ignored could cause serious issues at a later stage and hinted surgery at the sphincter. I’ve made changes to my lifestyle and I’m cautious of food I take (Ex: Less spicy food) and even then the issue keeps recurring. Pls review the reports and suggest if the surgery is the only option or are there any other alternatives.

Yes...pls refer XXXXXXX 2015 report. H pylori detected and I took medication for that.

Hi,
Thanks for the additional information.
Long term uncontrolled GERD can cause serious changes in the lower esophagus.
I suggest a biopsy be done from the lower esophagus to see whether any change has already and which will also help to plan the future management.

Surgery is the last resort in the treatment of GERD.
I suggest the following for controlling the following before going for surgery.

1. Weight reduction if you are obese. Try to keep your BMI bellow 25
2. Avoid alcohol and smoking (If you have those habits)
3. Avoid coffee, citrus, chocolates, carbonated beverages, peppermint, tomatoes, fatty food, spicy/masala food
4. Do not eat full stomach. Eat multiple smaller meals. Try to have 5-6 smaller meals per day instead of the conventional 3 larger meals
5. Stress reduction if the stress levels are high. You can meditate or listen to good music to reduce stress
6. Go to bed at least 2-3 hours after food. Keep the head elevated by raising the head end of the cot by about 6 inches.

Take C. Iladac - L once daily 15 minutes before breakfast and T.Famocid 40 mg once in the night before dinner for 8 weeks.

First try to follow the above steps and look for the response. If there is no adequate response, then surgery may be planned.
