Question: XXXX

I have a burning lower right shoulder blade pain.
I'm right handed. 61 years old. 170 lbs. diabetic type 2. had broken left wrist, elbow and collar bone 10-15 years ago. I had successful colon cancer surgery (12 inches of colon removed) 9 years ago.

I Play golf (about once every two or three weeks) . used to play baseball. Work seating down at computer.

Medications:
Metformin ER 500g 24 hrs. tabs 2 tablets twice a day.
Lisinopril 5 Mg 1 once a day.
Atorvastatin 40 Mg 1 table once a day
I have been prescribed Naproxen 500 Mg one table twice a day in the past to deal with chronic rotator cuff pain. Cortisone shots made the pain bearable and I’m no longer taking this medication.

Can you help?

Dr. Hanif warmly Welcomes you to my virtual clinic HCM!
I have carefully worked through your case, and can realize your health worries. Being your physician, I will take care all of your medical concerns.

This is all due to injury to the rotator cuff due to excessive use. Athletes and swimmers have this injury very common. Baseball and now golf are all that have done wear and tear to the anatomical structures of rotator cuff.

Management:-
The main steps involved in management are; Decrease stress and work load on the shoulder; Reduce inflammation; and if there is any Bio-mechanical issue to the joint that needs to be corrected e.g. any loss in the range of movements of the joint etc.

So stop Golf right away. Use shoulder sling to reduce mobility and make things better. 6weeks is the recommended period for rehabilitation process to work properly.
Apply Thermacare HeatWrap at shoulder. It will soothe you a lot.
Take a judicious amount of water and proper rest.
Avoid excess of coffee, tea soda and caffeinated energy drinks.

*Take Ibuprofen 800 mg twice daily for a week. Then once daily for a period of 5 more weeks. Don't miss a dose. Avoid taking on empty stomach and if stomach hurts add OTC Omeprazole too.
*Corticosteroid shots are once again needed as you already had. It will reduce inflammation and will help fast recovery.

Have a wonderful time ahead. Hope I answered all of your questions. Let me know if you have any further clinical concerns or doubts. I will be more than happy to get back to you in no time. If satisfied here, please give a generous five star rating and don't forget to close the discussion.
For any disease you have, a timely follow-up after sometime regarding the treatment efficacy and to see any need to change the treatment options is very necessary. Keep a follow up some days after this visit session ends.

Dr. Thank you for your reply.

My question is directly concerning my "burning lower shoulder blade pain" not
my rotator cuff. Reference to the cuff was only mentioned for your information only.

Is this burning lower shoulder blade pain related to rotator cuff injury? Please clarify.

Brief Answer:
Yes, these are interrelated.

Detailed Answer:
I welcome you back here with a follow up query and happily available to help you further with any doubts.

These pain episodes are all related to the 'rotator cuff'.
What is a Rotator cuff actually?
The human rotator cuff actually is the name of bunch of "tendons" & "some muscles" of shoulder, that connect our upper arm (humerus bone of the arm) to the shoulder blade (scapula bone that can be felt on the backside of shoulder too). In the rotator cuff tendons are like slings that provide stability and support to the shoulder joint as a whole; the muscles allow the shoulder to rotate in different direction for daily routine activities. There are four named muscles in this structure.

When we say that there is a rotator cuff pain or that a person has a sharp burning pain in lower shoulder blade; these are the one and same thing. Rotator cuff is the other name of the structures of shoulder that are under the hood of the shoulder blade.

Kindly be informed that, Rest of the instructions and suggestions are the same as aforementioned.
