Suggest Treatment For Breathing Difficulty When On Ventrolin

Question: Hello doctors,

Thanks for clearing my earlier doubts.

I need an advise related to pulmonology.

I have been taking ventorlin as first line of help to get rid of my breathing problem, just to share some more light my actual cause of concern as of now is sleep, I find difficult to fall asleep, once I sleep eveeything is fine but falling asleep is a challenge for me mostly I am anxious, I tried to find reason and found that ventorlin tends to have that side effects.

I did my ecg and echo it came back normal, ecg was borderline 800 sinus rythm normal, remarks: within nornal limits and echo was excellent too, pft test done 7 months ago said I have mild asthma, since late childhood I have this problem, in middle child hood it was not there and since last 6 to 7 yrs have breathing issues.

Its not like I gasp for air or having difficulty breathing, its the breathing uneasiness, but I doubt if breathong is playing a role for my sleeplessness I mean my breathing during night is not that bad, if you ask me to run at night I can run for few mins without any issues.

There has never been a situation for me where in I had to run to emergency except 6 yrs ago where local physician gave me nebuliser, but as of now I am using ventorlin, I heard it creates anxiety and that it may create sleep problems, please suggest a suitable one for me.

I dont have any triggers, its just that if I rest or stay idle in terms of physical activity the breathing becomes uneasy and hence I take ventorlin inhaler, my bp during sleeplessness or when feeling uneasy in breathing is below 120/77 and 115 to 118/65 to 85 on an average and heart rate is 60 - 85.

Breathing uneasiness occurs 7-8 hrs, for instance I am in office and working and am idle in terms of physical activty then the breathing becomes uneasy but not impossible or very difficult I can live with it, but I take ventorlin to relief myself.

Hello dear, thanks for your question on HCM.
I can understand your situation and problem.
Yes, you are right about ventorlin inhaler and it's side effects of anxiety.
It also causes tremors, palpitation and sympathetic over activity.
So this might be the reason for your sleep related disturbance.
Ventorlin is having levosalbutamol and it is short acting beta 2 agonist. Amongst all beta 2agonist , it is having higher side effects and more sympathetic stimulation.
Newer beta 2agonist are available which are having long duration of action and having very less systemic side effects.
Commercially they are available in the combination with inhaled corticosteroid (ICS).
LABA (long acting beta 2agonist) and ICS are considered as treatment of choice for asthma.
Both are very safe in long term use. Symptomatic control is excellent, so I advice you to start LABA and ICS combination instead of ventorlin.
You should also start oral tablet containing combination of antihistamine and anti allergic.
Antihistamine drug will also help you in initiation of sleep.
Since all these are prescribed medicines, you need to consult doctor for their prescription.
