Question: Hi Doctor, my wife (ago 40) was just diagnosed breast cancer, with BI-RADS 4c (by ultrasound) and BI-RADS 3 (by molybdenum target method). I want to know does she has to take Mastectomy surgery instead of Lumpectomy? The doctors here has not determin which method to be done, they need to cut out the tumor to check whether it's good or bad. (like biopsy but they do surgery at the same time)

Brief Answer:
breast conserving surgery depends on size of tumor

Detailed Answer:
First diagnosis of breast cancer needs to be confirmed by biopsy. Trucut biopsy, where a thick bore needle is passed and a piece is taken out for examination, can be performed for diagnosis.
After confirmation of diagnosis of breast cancer, staging workup needs to be done to check whether the tumor has spread to other organs. Most common sites of metastatic spread for breast cancer are lungs, liver and bones. If after doing imaging tests for staging workup if the disease is localized to breast region, when surgery is planned.
Breast conserving surgery can be done if the disease is small enough for the breast to be conserved. Small tumors in large breasts are ideal for such surgery. If the tumor is large, or located in central part of breast or if it is large tumor in small breast then breast conserving surgery is not possible and complete breast needs to be removed. The surgical oncologist attending on her is the best person to decide the type of surgery.
