Brief Answer:
Treatment depends on the grade of piles

Detailed Answer:
Hi XXXXXXX

Good Evening.

Thanks for your query, read the history of your problems , treatment and sclerotherapy, painless bleeding.

I hope you have been seen by a General Surgeon.
May I please know who examined you, did the sclerotherpy; in fact I am more interested to know whether piles is a confirmed diagnosis / What was the grade before sclerotherpy and what is the grade now? The treatment depends upon the grade, (for example grade I is not operated).

Another most important aspect of piles treatment is that, the treatment modality is always decided by the personal experience, preference of a Surgeon.
Also are the factors like the associated problems the patient patient may have- like colitis, stress, bowel habits, IBS, food habits, Working conditions and tensions of a patient. I hope you are getting my point.

I would like to know the grade of pile at the moment.
Any associated factors you have noticed or your Doctor has suggested ?

Treatment for cure can be divided in two parts
1. General
2. Specific to piles.

1.General treatment :
-Nutrition: Good hemoglobin, good serum proteins helps
-Diet : Whichever suits you well, should not cause constipation or loose motions.
You have to avoid food and beverages which alter your food habits. No special foods are necessary although in my experience taking ladies fingers ( Bhindi) cooked or raw gives good motion the next day.
-Stress and anxiety have a direct relation to bowel habits and related problems.
-Too much travel, odd hour working, late nights indirectly affect.
-Straining at motion for whatever reason.

This is a general outline and it is possible that some factors may be applicable in your case. I would advise you to rectify them as far as possible.

2. Specific:
-Sitz bath- in a big tub with warm water before (if there is no urgency the moment you get up) and after passing motion.
-In grade I and II piles a gradual anal dilatation by a General Surgeon may help to reduce the congestion and indirectly help for cure.
-In some grade II and III piles the best treatment is a classical excision and ligation of the piles at the root of it give the best result. If you maintain the general factors I mentioned, it will possibly be cured and you may not have recurrence.
-Failed sclerotherapy is another indication for a formal Surgery.
-Avoid too many medicines if they are not helping as they alter the local conditions. For example too much laxatives gives more trouble and has a problem of habit formation, soiling and the doses have always to be altered.

I hope this detailed answer will help to take a proper decision, give you the best and permanent solution.

Please feel free if you have further queries or clarifications. I shall be happy to assist you more.

Brief Answer:
Get Examined by a General Surgeon.

Detailed Answer:
Hi XXXXXXX

I can understand what your family members must be thinking. It is true that the treatment of the piles is done by all pathies Allopathy including surgery / Homeopathy / Aurveda and may more claim great results. The problem is that it is difficult to say who did what. So one hears about the failed treatment.

You can certainly find a XXXXXXX General Surgeon in XXXXXXX who has a good reputation about piles cases. The one who does a classical Ligation and excision at the root of the pile mass can give the best results. This is for sure.

I will try to locate someone in XXXXXXX

Please follow the general outline as I have given.
Get a consultation of a XXXXXXX General Surgeon, know the exact grade of the piles, discuss in details about the outcome of surgery and please give me the follow-up, I may be of help to you to take a proper decision.
