Suggest treatment for blood discharge from anus

Question: I have gas with bloody red discharge anally. When I urinate is hard because of gas. Straining to poop. Mainly gas and spotty blood. Pain lower right middle back. If someone slaps me on the back it hurts right middle back painfully

Probable causes:-
It may be either; ANAL FISSURE or HEMORRHOIDS.


Both of them have many things in common. Treatment and home remedies for both are same and if it will be an anal fissure, you will get better even without ever landing into the doctor office. It is because mostly anal fissures heal up by themselves and nothing much is needed to do for them.


What happens is:
Due to constipation patient has to strain and the hard stools makes cut on the anal verge and mucosa of the anus. This is called anal fissure. It is painful and bleeds too.


Treatment:-
As anal fissure is a tear in the wall of anal mucosa and due to pain the muscles of the external anal sphincter becomes very tightly closed as a guarding reflex to avoid more pain on straining and passing stools. Mostly patients avoid voiding feces due to pain.


First and Important Step Is:-
The first step is to manage the pain and improve his bowel movements. The main issue in such a case is with the unbearable pain during the act of passing stools and that is a reason for avoiding and holding feces and it leads to harder stools making the situation even worst.


To Relieve the unbearable pain:-
Take Ibuprofen by mouth and ProCort rectal cream (to be applied with an applicator inside the anus).
Petroleum jelly will also be beneficial alongwith the above cream.
If stools are harder please use Adult Glycerine Suppositories for loosening it up and making the situation easy.


Sitz Bath:-
Use Sitz Bath 3 times a day to soothe and heal. It is actually a bath tub with XXXXXXX warm saline water in it dipping the hips when sitting in it. Most of the anal fissures heal up by themselves with these home remedies and the above cream and a very few needs surgical care or Botox injection into the anal sphincter muscles.


Instructions:-
Take upto 6 litres of water+other fluids.
Take Citrucel dietary fiber supplement with plenty of water.
Syrup Lactulose at night for stools to get soft and become of proper bulk.
Take more vegetables, fruits and salads in diet.
No beverages, caffeinated drinks, energy drinks, refined foods, fried and junk foods and processed meat.
Take whole grains.



If the situation doesn't get better even after observing all the above instructions, consult a Gastroenterologist for Colonoscopy and Proctoscopy to see what is going on in there. He is the personnel trained to treat such cases successfully. He will diagnose the case and will rule out if Hemorrhoids or Ulcerative colitis may be a reason.

Yes, it seems to be the anal fissure as I explained in detail previously. It causes the symptoms you feel.

But sometimes, the symptoms overlap in other diseases like Ulcerative Colitis, IBS-C, Some lesion in the colon or rectum, and Hemorrhoids (or piles).

Start the remedies and the treatment as prescribed above and I am sure you will get better very soon. But if in case, you feel the symptoms again you need to see a specialist Gastroenterologist for proper and detailed evaluation with diagnostic tests to see the cause of your disease.

What about the gas i am having. I lay down,sleep and get up to go it is like all gas.

Yes, the gas is produced due to the slow motility and the improper bulk formation of the stools. Also the constipation contributes to the gas production and the bacterial growth causing gas has more time to work due to slow motility.


Once you start the medical treatment aforementioned, you will better regarding gas too. You may add Levosulpiride+Lansoperazole (before breakfast and dinner) if in case you feel there is still gas disturbing you after the above remedies. It is a very good medication to help assist digestion and maintaining the normal levels of the stomach acid.
