Suggest Treatment For Blocked Tube And Multiple Miscarriages

Question: Hi,


A little history abt me -I am 37.5 years old, I had PCOD, 1 tube blocked, multiple miscarriages, high thyroid for which I take thyronorm 50 daily (Thyroid is 5 these days). I have 1 child born 4 years back and delivery was c-section.


I had visited gynac last month to get IUD implanted. During implantation she told me that she can see some endo polyp and she took out a small lump and suggested me to get its cancer screening done (which I gave in lab for testing right then).

She usually asked me few symptoms like heavy bleeding, spotting, irrregular periods which I answered in negative.

After next periods she told me to get the ultra sound scan done and meet her with both ultra sound scan and other report.

1. Cancer screen report - negative
2. Ultra sound report -
     Observation : Transvaginal Ultrasound done under 2 Dimensional spectral colour Doppler. Cervical polyp measures 6 mm     Uterus measures 7.9 x 4.6 x 4.7 cm vol 99 ml IUCD insitu Uterine contour normal and Uterine cavity echo demonstrated The endometrium measures 0.45 cm Right ovary measures : 2.4 x 2.7 x 1.4 cm vol 5 ml Left ovary measures : 2.6 x 2.8 x 1.7 cm vol 7 ml No free fluid in the pouch of XXXXXXX

     Impression : CERVICAL POLYP IUCD INSITU Ultrasonography is the study of images hence not 100% accurate. Serial scans and reconformation is recommended. the reports are not valid for medicolegal purpose . Clinical opinion needs to be taken.

I got busy with my work so could not consult her immediately. its been 16 days sunce ultra sound happenned, and technically today is the 22nd day from last period. I am finding following observations:
     1. last 5 days small red blood tissue being discharged
     2. Mild discomfort in abdomen and mild bleeding in last 2 days
     
Can you pl. suggest if the reports are not fine and what should be my course of action.

I have gone through your reports and it is showing cervicervical polyp of 6mm in size.

Now you are having symptoms which may be due to polyp.

It is basically a benign growth but if it produces symptoms ( as in your case), polypectomy is advisable.

So you should consult a Gynaecologist and get a polypectomy done( to remove polyp).

I have a follow up question. What is polypectomy ? Is it a surgical process ? If yes, then how major/minor surgery is ? If the surgery is not performed, what are its implications ?

Can this be cured using medicines ?

Yes, polypectomy is a minor( OPD) surgical procedure to remove polyp.

Usually if polyp is associated with symptoms, it is better to removed it, because it may cause discomfort and in 1-2% cases it can be cancerous also.

No, there is no medicine to treat it but sometimes it may fall off by its own during menstruation or sexual intercourse.

After removal it should be sent to pathology to see for any cancerous cells.

In your case as it is causing symptoms and polypectomy is a very simple procedure, I would suggest you to go for it.

I would like to ask more about the surgery . Is it done on some anesthesia ? How long does it take to recover ? any side effects/precautions ?

It can be done under local anaesthesia.

Sorry for the long delay in my response.

This procedure can be performed in OPD without anaesthesia or sometimes local anaesthesia is given.

If cryocautery done after procedure than there is a watery discharge for 2-3 week, so you should not drive immediately and no sexual intercourse for 3 weeks.

Complications usually not occur but sometimes infection and bleeding can occurs.

In short it is a routine procedure done in OPD.

Thanks again for the reply.

I met a gynac yesterday and she did an internal examination. As per her she did not see anything superficial that could be taken out in OPD. She told me that it seems the polyp is internal and we should go ahead with the surgery.

Do you suggest me to wait for a while maybe 2-3 months and then decide for a surgery ? As per one of your earlier suggestions "it may fall of on its own"

Yes, in my opinion if you are not having severe bleeding or any other symptoms due to polyp, you can wait for 2-3 months.
