Suggest Treatment For Blocked Nose

Question: Hi

I usually have blocked nose. Feel something inside. I have to blow multiple times and with heavy pressure.

I seems to have this from my childhood.

Hi,

Thank you for your query.

1. Nasal turbinate hypertrophy or nasal polyps, in addition to DNS is a leading cause for nasal obstruction.

2. Get a Sinus CT done. Ideally the CT Scan should be with 1 mm sections, before and after de-congestion with 0.1 % Xylometazoline nasal dops (such as Otrivin). After the first scan, use 5-8 drops of Otrivin in each nostril, wait for 15 minutes and re-scan. The radiation settings should be set to below half. Compare the images with any improvement after the drops. This helps identify the site/s causing the nasal obstruction.

3. If you can share the CT Scan images, I will be able to give you an accurate assessment.

4. You should take a course of medication before the CT Scan. Medication should include an anti-allergic (such a combination of Fexofenadine and Montelukast) , a steroid nasal spray (such as Mometasone) and regular steam inhalation. I recommend that you take a trial of at least two weeks with this medication.

5. Revision surgery should be done only after a full trial of medication. Radiofrequency / Coblation assisted nasal turbinate reduction and Balloon Sinuplasty is a new option. Nasal polyps require endoscopic sinus surgery.
