Suggest Treatment For Bloating, Constipation And Constant Belching

Question: Male, 25, 5ft10in, 67

I have been suffering from these symptoms from last 10months:

1) Incomplete evacuation. I think my stomach never emptied since last 10months.
2) Bloating (due to months of incomplete evacuation??).
3) Sometiems I feel something is moving in stomach.
4) I go to toilet daily and sometimes 2times a day.
5) I spend too much time in toilet. This is what happens during stool passage- Some stool comes out, then some gas comes out, that some stool, then gas and so on..... I think gas is trapped inside stomach.

- Constant belching/burping from last 5years. I can burp on commad. Its like some air is always there (just below the throat). That means I am always ready to burp as many time at any time of day.
- No pain in stomach. But stomach fullness is discomfortable. No pain during stool passage.
- No hard stools (or may be sometimes).
- No difficulty in stool passage (or may be it happens rarely).

"THIS ALL STARTED DURING MY EXAM IN MAY- AT THAT TIME I was sleeping 4-5hours a night, I was under stress all the time. And I was studying more than usual. Almost no physical activity."

I am under stress from last 1year due to personal resons. And sometimes I got anoxious very badly and when I got anoxious it feels like someone is squeezing my stomach from inside.


Please clear my doubt weather it is IBS or Chronic constipation or GERD or anything else?
These are the things I will do from tommarow:
1) Exercise daily for 40minutes.
2) Drink 8-10glass water.
3) Cuttoff tea to 3times/day.

But first I need to expel all this accumulated garbage from my body.

I can understand your problem. Your symptoms got precipitated during stress (during examination) and you have repeated stress because of personal reasons and have problems of anxiety also. The feelings of incomplete evacuation, bloating, abdominal discomfort, repeated thoughts about bowel movements etc can occur due to Irritable Bowel Syndrome (IBS).

In chronic constipation or GERD these symptoms are not usually seen. Occurrence of these symptoms along with stress also further support diagnosis of IBS. In GERD epigastric discomfort is main presenting complaint and usually individuals don't show complaints related to bowel. In chronic constipation such symptoms can be seen but occurrence of stress favour more towards IBS.

Apart from the life style changes you have mentioned I would also advise you to increase fibre intake in your diet. Try to avoid saturated fats in diet. Eating a cucumber daily will help to soften stools and will result in good evacuation. Try relaxation exercises and avoid anxiety. If even with life style changes symptoms remain then visit a doctor for help. In IBS medicines like Clinidium Bromide and Chlordiazepoxide combination results in significant improvement. Some time low dose antidepressant drugs as escitalopram also prove useful. Visit your doctor for the prescription of these drugs.

Thanks for quick answer. Here is my follow-up question.

1) Are you sure this is IBS? Because I think I mixed Stomach fullness or bloating with discomfort. I never had/have pain or cramp in stomach. I feel like my stomach is full due to the incomplete evacuation and accumulation of 10months load of garbage.
And excessive anxiety can also cause constipation and stomach cramp?

2) I have read somewhere on internet that the only thing that differentiate chronic constipation from IBS is abdominal pain or cramp. And I dont have pain or cramp.

3) Can your prescribe me any medicine to clear my stomach and gas?

- From the history you have mentioned and with symptoms I can make provisional diagnosis of IBS. Fullness, bloating and stomach discomfort are important clinical symptoms of IBS. It is not necessary to have pain in stomach. Association with stress further support our diagnosis.

- Abdominal discomfort is considered as pain in making diagnosis. Severe pain doesnt occur in IBS. Secondly if this was chronic constipation it would have relieved with stool softeners. But that probably not happened.

- As this is a online platform and due to legal hassles we cant prescribe you medicines online, also the pharmacist will not accept the online prescription. You have to visit a doctor for proper online prescription. I have suggested commonly used drugs in IBS. You can discuss with your doctor for these drugs.

Thanks for reply.

No sir I do not have/had any pain/ache/discomfort/cramp in stomach. It is fullness and stomach distension(half inch).
And I feel fullness in my upper stomach, I also hear some sounds from stomach like some liquid in the upper stomach. And also there is too much gas trapped in my stomach.

If it is IBS despite the fact that I don't have any pain/discomfort/ache/cramps, Then can you please tell me the following:

- Is this condition permanent? or It can be treatable?
- Now, is anything can be done to empty/clean my stomach or it will remain full and gaseous?
- Is my constant burping problem(that I have from 5years) treatable?
** The constant burping problem started after excessive vomiting when I was ill.

- Can healthy living(no stress, good food, exercise, 10g water) improve this condition?

Yes the history goes more in favour of IBS, please don't be stressed due to that. Now coming to your doubts-

- No the condition is not permanent. It is treatable with medicines and life style changes. Even with relaxation exercises and control of anxiety symptoms improve.

- Life style changes especially drinking a lot of water, increased fibres in diet, reduction of saturated fats in diets etc will result in good evacuation. Taking bananas daily also help in evacuation.

- Yes constant burping is treatable. It has linked itself to stress and anxiety also. Antacid medicines with medicines for IBS will control the burping.

- Yes as I have mentioned earlier also, life style changes will improve the symptoms.
