Question: 77 year old male with diabetes has rash on upper arms especially the right one, rash with swollen testicles, and blisters (big ones)on his right toes----care to hazard a guess

Rashes over upper arm, over scrotum (swollen skin due to rash?), blisters over right toes following a recent introduction of a new drug made me to think of the possibility of 'Fixed Drug Eruption'.
It is type of allergic reaction to some drug. It will heal on withdrawal of that drug, but to recur at the same sites along with new sites when exposed to that particular drug once again.
Systemic steroids and antihistamines are necessary to treat the same.
If you can upload the picture, it would be helpful for further discussion.

will try to give you clearer picture without actual picture because i am a bit computer challenged. Enflamed testicles started Tues. and new drug(tradjenta 5 mg. daily) started about 8days ago. Rash on upper arms came on Wed. morning and large blisters on toes showed up today. Was just one blister on second joint of one toe and then two more blisters a few hours later on two more toes and another blister on tip of big toe with blood in it. Stopped possible suspect meds yesterday. The rash on upper arms is red slightly bumpy area that itches. Mostly onthe underside of upper arms with a small patch on forearm near the elbow. patient has diabetes and has had a triple bypass. does not feel bad other than testicles are very sore and no fever. started antihistamines last night (benadryl). cannot think of any other changes in lifestyle

Testicular swelling along with blisters over toes and rashes over arm, we have to think of a viral infection affecting testes with exantheme (rash).
Blisters over the toes fungal infection of foot has to be ruled out. A careful examination by a Dermatologist along with investigation can clinch the diagnosis. Testicular swelling needs anti-inflammatory analgesic drugs. Consult your physician or Dermatologist.
