Suggest Treatment For Bleeding From Rectum After Masturbation
Question: I believe I perforated my rectum, or have a anal fistula. I was intoxicated looking at porn and was curious and experimented with a foreign object. At first I had a bloody mucus coming from my rectum, but I don't anymore. My stool was almost black, but it's not anymore. Then I had huge gobs of clear mucus coming from my rectum when I urinated, but I don't anymore. I just have mucus in my stool, and some mucus comes out when I push to hard. It feels that I'm leaking fluid into my rectum, and its secreting out into my crouch reign. I have pain in my rectum, but not when I push out a stool. When I feel the urge to defecate I cant hold it in I have to go to the bathroom right away or I'll crap my pants, and when I do I'll only pass gas and blow out small amount of excrement and mucus. when I do defecate its a very small amount, and there's penciling of the stool. I saw a doctor to get a referral to see a gastroenterologist to do a sigmoidoscopy, but I wasn't entirely true with her. I didn't tell her about the experimentation, I just told her that I was constipated and took a bunch of laxative and now I'm having these systems. I asked her if I should go to emergency, she said no. she said would refer me to a physician to do some tests before he sent me to see a gastroenterologist for a colonoscopy. But now I'm starting to develop a fever, and I'm having pain in my liver and kidney. I don't know if I can wait that long, I'm afraid my condition will worsen before I see him. I don't know what I should do. should I go to emergency, or wait to see the gastroenterologist. can you please advise me, what I should do.

Read and understood your history.
Noted the history of experimenting with a foreign object, has blood, mucus, black color stool, huge gobs of mucus while urinating and these have stopped. Now there is only mucus in stool and on pushing hard.
Also read about the Pain in rectum, urge to pass but passage of only gas small amount feces and mucus, can not hold.
Less than the truth with your Doctor and could not get the referral to GI but to a Physician, But now there is fever, pain in liver and kidney; want to know the next step.

Can you please tell me what exactly was done?
What was inserted ?
Was there any pain while doing the act?
Or active bleeding at that time?
When did you do this?
Can you move around and change position in bed without added/increase in pain?
I think you should go to the Emergency, tell the truth to the attendant their so that the most important investigations will be done to find out any harm is done or not. Getting a correct diagnosis is very important.
I would suggest you the following and discuss this at the Emergency room so that they do it at an urgent level.
- X-ray of the abdomen in standing position For gas under diaphragm: this is the simplest investigation to give the maximum information.
- CT scan preferably, to see any trauma, collection in the abdomen and so on.
- Colonoscopy is actually not done immediately in the fear to perforate or to spread infection, hence not to be done, it is in fact dangerous, hence not necessary to go to the Gastroenterologist at the moment and getting an appointment may be a problem.
- Tests of blood, urine and stool will help further.
- The most important is the clinical examination by a Doctor who can elicit a classical sign of rebound tenderness and have thorough physical check-up.
Actually in practice: the actual perforation of the rectum or colon is so painful that you can not move at all, you can not talk, can not type. This is one pain which is very classical; if there is no very severe 10/10 pain, do not worry.
Best way is to rush to ER and tell the truth so that you get the best possible diagnosis and treatment.

In response to your questions, i was intoxicated looking at pron then got the urge to experiment with a foreign object. It was a stick (wooden knife) used in material arts training, yes there was pain while doing it, yes there was bleeding (bloody mucus) at the time, it was done 3 weeks or longer, yes i can move around in bed without pain. i have pain in my rectum but its not 10/10, its about 5. I don't have pain when passing stool, just when i'm sitting. I have mucus coming from rectum when i push hard, and in stool. I have hard time holding in my stool, when i feel urge i have to go to bathroom right away or i'll crap my pants.

so your saying i don't have to go to emergency? I also have what feels like my anal glands are leaking into my rectum, secreting out through sphincter and scrotum my underwear always feels damp in the crouch area and when im feeling the leaking, it feels like a wet sensation going into my but ox. I was told earlier by another doctor on line Dr. XXXXXXX Steinberg, which i was also not entirely honest with about the situation, that i possibly have a anal fistula. She was the one that advised me to get a colonoscopy can you please advise.

Hi Dr. Chandrakant, thanks for your email and your concern. Nothing has changed since I talked to you last, I’m still having all the same symptoms. I haven’t seen a specialist yet, I went to emergency about two weeks ago. They were going to set me up to see a specialist two weeks ago, and I finally got a call back yesterday I’m supposed to go for a colonoscopy on the 26. I said to the lady I was talking to what you said that a colonoscopy was the worst thing to do, and she said who said that and I told her. Then she said that’s not true, so I don’t know what to think can you advise?
