Brief Answer:
Need further evaluation.

Detailed Answer:
Hello
Thanks for your query, based on the facts that you have posted it appears that you have obstructive voiding symptoms due to Bladder Neck Obstruction which was opened up by doing Trans Urethral Bladder Neck Incision thrice during last 7 years.
I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Was following basic tests done before doing surgery of BNI
a) Ultrasound Scanning of abdomen and pelvis for Size of the prostate and to
know post void urine volume
b) Uroflowmetry to study to know the voiding pattern whether weak or normal

Since you have undergone surgery of BNI thrice your obstructive voiding symptoms could be due to either of the two following reasons
1)Narrowing of urinary passage (Stricture Urethra) developed due to repeated
instrumentation
2)Weak bladder contraction insufficient to pump urine outside

Please consult qualified Urologist and get following basic tests done to find out the cause of your symptoms .
1) Retrograde Cystourethrogram to rule out having stricture of Urethra (Narrowing of urethral passage ) .
2) Urodynaemic study of the bladder .

Further treatment will depend upon the results of these tests and final diagnosis.

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.
