Suggest Treatment For Bipolar Disorder

Question: Dear Doctor,
My beloved wife is suffering from Bipolar disorder( Schezophernia) since more than 16 years. I have tried my level best to get her best available medicine. But unfortunately I couldn't succeed in curing her. And she us very much reluctant in taking oral medicines prescribed by reputed Psychiatrist in my city. Currently I am getting her administered Clopixol a Depot and Clopixol Accuphe. Earlier on four occasion I have given her Resperadal Constant also. She is a diabetic also. She is refusing to take medicines for that also.Now
my life has become miserable because of her behaviour. The atmosphere in my bungalow is very much disturbed.
I am helpless and mentally tired. So please give your valuable advis. With high regards. Mr. XXXXXXX

Thanks for writing to us. I can understand your situation as a husband of such patient. Schizophrenia and Bipolar disorder are different disorders and course, prognosis and treatmet of each differs. However both disorders almost not curable but controllable.

From your narration of medicines I think she has schizophrenia. In schiz also negative, positive and catatonic types. I request you to provide her past and current symptoms detail so I can guide you about treatment.

If you can provide me her education detail, condition of her when illness started , family history, her compliance of taking medicines and history of other treatment like ect , I can also specify prognosis.

Dear Dr. XXXXXXX Solanki,

Thanks for your reply. My wife is a commerce graduate. The mental illness started after the delivery of my fourth daughter on 7th February 1997. I immediately took her to a reputed Psychiatrist at my place for diagnosis. The Doctor told me that she is suffering from Paranoid Psychosis(Bipolar Disorder). There after on Doctor's advise I have started giving her medicines and sometimes injections as well on periodic manner. She became diabetic during 2007. Now since many years she is reluctant not to take any medicines. And I am calling a nurse from a reputed hospital on every fortnight to administer Clopixol Depot and Clopixol Accuphase as per Doctot's prescription. Even that is also not giving any results. Me and my daughters are exhausted in controlling her. She is scolding maid servants, me and my children by using unparliamentary words. She suspects everybody that they are bad for her and we are not caring her and always worried about her gold ornaments and money. Infact I have secured everything for her. I am fed up of my life. Please advise me about the best treatment available in this world. I don't mind taking her to overseas for any brain surgery if it is required. I heard that in XXXXXXX USA in one of the world best hospitals, they inject genes to brain stem cells for this kind of

Hello Mr. XXXXXXX XXXXXXX

I can understand you situation.Just understand and accept what I want to say and move forward.

From your narration she seems to be suffering from Paranoid Schizophrenia.Her specific problem is illogical and false doubts which she believes are right.(It is called persecutory delusion)And secondary to these doubts all problem arises in different area of life and with different persons.

As you might be from XXXXXXX I guess you have consulted well known psychiatrists and most of the medicines have been tried. In such types of patients,major drawback is that they are not ready to take medicines as they believe they do not have any problem.

From my clinical experience I can say when person is educated and her main problem is suspiciousness only without any cognitive disturbance, it is really difficult to treat.

You can have following options.

If she is ready to take medicines, zyprasidone 20 mg to start with and every week increasing 20 mg up to 80 mg and other is trifuloprazine(5mg)+ trihexyphenidyl(2mg) combination once in a day to start with and increasing every week up to 3 -4 times a day depending upon tolerability and response.In many resistant patients, I have found response.

If she is not taking medicines on her own, you can give it by mixing in food. you can mix trifluoperazine combination but not zyprasidone in this way. So you can use risperidone liquid 3-6 ml per day by mixing it in food or drink.With these continue depot injections which you are giving currently.

Another option is ECT , if it is not given yet.It might help.

There is not at all any evidence of role of surgery or genetics in treatment of such disorder right now anywhere in the world. Please do not waste your time and money in that.

If her problem is so severe that it is danger to her or family members , she can be admitted to hospital for mental health and can be given treatment forcefully according to our laws.

Now the most important thing.Frankly speaking no specific treatment for such patients, which is curative, is available in current world right now. There are lots of family members of such patients like you who are suffering with the patients.We can currently only control the disorder not cure.Please understand and accept this reality and make yourself strong and motivated to face the situation.Your aim should be to control her anger and behavior disturbance and all family members have to understand this also.Whatever options I mentioned, you can try. If all of above options had been tried, late me know.Also send me your confirm city name, so I can suggest some psychiatrist to whom you can consult.

I really appreciate your attitude and caring nature toward her.Very few husbands of patient with schizophrenia are with such nature.May God giver her much recovery and give you and your family ultimate strength to take care of her.

In her case no role of Neurosurgeon at all if her MRI Brain is normal. If she had not undergone investigation MRI brain, please go for once. Mostly it will come normal.
