Suggest Treatment For Bile Duct Cancer

Question: Hi my husband has bile duct cancer. A stent was inserted 12mths ago. Whipple procedure started but aborted due to low oxygen levels. Radiotherapy/chemotherapy combined was administered followed later by more chemotherapy. Last week the stent was blocked and he has been feverish, vomiting. All fever stopped but still nauseous. He is extremely jaundiced and I need to ask if the blockage needs to be unblocked and a new stent inserted asap as we have now been waiting 5 days for an answer (he is in hospital) and he seems weaker every day can't eat, drink any movement exhausts him. He is not on any drip although it was suggested this morning this would happen. We are still waiting. I am as you can imagine incredibly worried. I would be extremely thankful for your opinion.

I reviewed facts of your query,in context to your health issues.
I also reviewed your worry currently.

My Opinion-

In My opinion, in the given situation, your husband has survived for almost 12 months after bile duct stent, where in this stent has blocked with extreme jaundice.

His general condition is also very weak and prognosis is gloomy in such a situation.
When doctors attending are not going for re-stent after blocked bile duct stent, it could be due to the grim body condition, where they are doubtful if he could withstand re-stent procedure.

Do's Suggested-

In this grim condition,I would suggest to use percutaneous biliary drainage by doing Percutaneous transhepatic bile drainage, if this facility is available.

This is the only option which could be used to improve the extreme jaundice in given situation.
Cover with antibiotic would be needed to control biliary infection from blocked biliary stent.
This needs to be done in Consults with treating /attending GI specialist doctors there.

Next appointment-
Asap- to review treatment progress .
