Suggest Treatment For Bilateral Ovarian Cysts
Question: Hello Doctor,
My wife has been diagnized with 2 bilateral cycts. One is a simple cycst in left ovary and one is a complex cyct on right ovary.
The measurements of simple cycts is 4cm and the other complex one is around 5cm by 3.5 cm
Please advise:
- if it is possible to concieve or if we have to shrink the cycts by medicines then try to concieve? Or should we remove the cyst by operation then concieve ?
- is there any natural foods you recommend that will shrink the cyst for eg: carrot juice?
- there may be a possibility that due to the size of cyst , she might not be ovulating. Is there any way out we can find out if she is ovulating or not?
- any other tips?

There are 2 cysts one on each ovary , one being simple but 4 cm and the other complex cyst with probable internal separations of size 5 cm above.
My concern is for the complex cyst which may be an ovarian dermoid cyst . This has to be confirmed with a CT SCAN and tumour markers to rule out any ovarian malignancy as well which have a similar presentation on ultrasound. So suggested as:-
1. get tumour markers done---CA-125, CEA, AFP, BETA HCG.
2. CT SCAN PELVIS to see dermoid components .
3. If dermoid confirmed and no ovarian cancer detected , then there is no other way but to get the simple cyst and the complex one probably dermoid removed by a laparoscopic removal before you even contemplate pregnancy.
These cysts shall not resolve with diet or rest or with natural methods or medications.

My apologies, the complex cyst is endometriotic. The other one simple. We didnt do any cancer markers but we did ultra sound.
- please advise if we should try to concieve? Or we must remove this surgically then try to concieve?
- please advise if there is any way to find out if my wife is ovulating? Any as in any ovulating kits or tests

if the other complex cyst is endometriotic for sure then tumour markers can be avoided but considering the size of both the cysts they have to be operated upon first before you try to conceive as the cyst size per se can hamper ovulation.
ovulation kits can detect LH SURGE AND not ovulation . ovulation can be detected confirmatively only by serial follicular monitoring with transvaginal ultrasounds.
it is suggested that as she has endometriosis as well , so a diagnostic laparoscopy and bilateral ovarian cystectomy should be performed first and if any other endometriotic lesions found in the abdomen they can be fulgurate as well to improve chances of conception.
