Suggest Treatment For Big Blister On The Glans Of Penis

Question: Hi, I had a big blister on the glans of my penis. It opened and the covering skin is gone. Now I have trouble taking care of it. Seems that the part in the middle is nice and dry, but I'm bleeding at the edges, and the area around the blister seems a bit swollen and red. I'm afraid things are getting inflammated. How should I treat this? Not sure whether to keep it dry or to apply desinfecting ointment, bandages etc. Can you advise? Thanks!

I have gone through your query.

I would like to know more from you regarding your query to ascertain the underlying cause of blister.

-Can you upload an Image so that I am able to comprehend the situation better?

I would like to rule out a drug eruption.
-Were you recently on some oral medications before the blister formed? like an oral antibiotic, pain killer etc?

Bullous fixed drug eruptions can present with mucosal lesions e.g Oral and Genital mucosa.
-Is there any oral involvement or any skin lesions elsewhere?

Just clean the erosion with a sterile gauze dipped in betadine solution.
In bullous drug eruption, a topical steroid+antibiotic combination ointment would be more helpful.
If drug eruption is indeed the underlying cause, a course of an Oral steroid would heal the erosion faster.
Otherwise, using just a plain topical antibiotic ointment e.g 2% mupirocin ointment, twice daily for a few days would also help.

Precautions:
-No need to bandage.
-Replace prepuce after applying the ointment

The blister is from a "traction device". I have mild peyronie, which seemed to be getting worse lately. I read on the internet that this device could correct things a bit (phallosan, works with a vacuum). After one night, I had an enormous blister.

I'll include a photo.

The black spot is clotted blood.
At the edges, the skin keeps breaking, it won't heal (I got the blister a week and a half ago).
In the middle, I seee some blood every now and then as well
Arounf the blister, the skin is a bit swollen and red

My followup questions:
- It seems the ointment is preventing the skin to close... It stays "moist". Should I just let it dry?
- If I have inflammation, should I take something?
- Other than that, based on the photo, do you have advice on how to treat it?

Hi.

Thank you for providing additional details and thank you for the Image.

It is a big erosion and its has some superimposed bacterial infection as evident by yellowish crusting (dried exudate).

I would suggest you to take an Oral antibiotic for a week.
My choice would be either a penicillin group of antibiotic e.g cloxacillin or amoxycillin Or you could also take one from the cephalosporin group e.g cephalexin or cefadroxil.
Oral antibiotics would be able to resolve the infection faster,

Clean it twice daily with a sterile gauze dipped in betadine solution.

Topically you may use an OTC antibacterial ointment e.g mupirocin or neomycin, twice daily, after cleaning with betadine dipped gauze

Follow these measures and it should be fine in a week.

Thanks so far, I ordered the medication. Should be coming in today.

What dose of oral antibiotics (I ordered cloxacillin) should I take daily?

Also, today, the open blister looks very dry. There is a lot of crusty, clotted blood at the edges. The middle part is still smooth and pink/red, also dry.
Is that a good thing? I'm not sure what it is supposed to look like when it heals.

Hi.

Ok. Good. Oral antibiotic would be better effective.
Crusting is actually due to dried secretions/ discharge from the wound floor at the edges.
Crusting is a sign of infection. It is not usual for a wound to discharge for a week or so without having some infection in it.

Topically you may use an antibacterial ointment which would not only keep the crusts moist but also being antibacterial would help in resolution of infection.

However, I am sure your ulcer would be fine within a week of Oral + topical antibiotics.

I advice you to keep your primary physician informed about the Oral antibiotic that you have ordered.

Ideally you should get a prescription for an oral antibiotic from your primary physician.
It is unethical for me to prescribe a dose of antibiotic online and I have to stay within my limits.
I can only give you options and pointers in the right direction via this form, which I did.
