Suggest Treatment For Bad Acne And Dark Spots On Face

Question: i have this acne problem break out sometimes my face is clear for like one month after that some deep deep pimples just appear on my face sometimes very painful and this actually affects the right half side of my face and leaves behide very dark marks....i just use deriva cms gel bt if i stop using then the acne props again..what do i do

I have seen the picture that you have uploaded.

From your history and photograph, I would consider grade 3 acne vulgaris. There are 4 grades of acne depending on the severity and you seem to have moderately severe acne.

I would suggest the following measures which you could discuss with your treating doctor/ dermatologist-

1. If I were to be your treating dermatologist, I would start you on a tablet called as Isotretinoin. This is very effective in treating acne but has to be strictly monitored by your doctor.We usually do a bood test to check the liver function and lipid level before starting the tablet and also every 4-6 weeks while on the tablet. Pregnancy should be strictly avoided while on this tablet.

2. You could also benefit from taking weekly courses of oral azithromycin for 6- 8 weeks.

3. You could apply a gelcontaining 1 % Clindamycin phosphate in the morning on the pimple prone areas.

4. Apply a gel containing a combination of 2.5% Benzoyl peroxide and 0.1% Adapelene at bedtime on the pimple prone areas.

5. You could get salicylic acid peels done every 2 weeks for 6-8 times. This will help in clearing the acne as well as the marks.

6. Use a face wash containing 2% Salicylic acid to wash your face.

7.Avoid cosmetics with a creamy base. You could apply a gel based moisturizer.

8. Drink upto 2 liters of water everyday.

9. Avoid fried foods and sugary foods. Eat lots of fresh fruits and vegetables.

Hope these measures help you.

Acne is a chronic problem which is hormonally mediated and which will subside by 28 -30 years. Until then , you need to use maintenance treatment like the gels I had mentioned above even after complete clearance is achieved. The above treatment will definitely help you achieve good results. But even after the acne is completely cleared, to prevent new breakouts, gels and face wash must be continued.

Can i use the tablet (isotretinoin) just from the the chemist without necessarily going to the doctor...You are a doctor already...or is there any need? i mean i can go and get the stuff that u've prescribed for me from the chemist and i will follow your instructions....would that do any harm without seeing a doctor first?

Isotretinoin is a prescription medicine. It cannot be dispensed without a doctor's prescription. Moreover, it has to be taken with the dermatologist monitoring you personally. Blood tests have to be done regularly.
