Suggest Treatment For Bacterial Vaginosis

default
Posted on Mon, 17 Nov 2014
Question: A few weeks ago I had sex quite a few times with a stranger and there were occasions I was dry and the sex was frequent. I was extremely sore. I also developed a bacterial infection and when I checked the next day I noticed the base of my vagina was swollen and my discharge smelled bad. I went to the dr. And they diagnosed me with a bacterial UTI. When I went home this was perhaps a day or two after the sex and I noticed a cut not on my perineum but a little above it, on the edge of the entrance. It looked like a little cut/gash. I was kind of worried because I checked the area the day after sex and it wasn't there or I hadnt noticed it then, so I freaked out and immediately thought herpes as it showed up later. It didn't hurt and simply looked like a tiny gash. I am concerned because I usually get shallow perineum cuts after sex that heal in about 2 days. This however took about 4-5 days to heal and was closer to the softer tissue on the edge of the entrance. It didn't hurt only when I went to the restroom and urine got on it. I'm not sure if bacterial vaginosis combined with the trauma from sex contributed to the cut/gash. I finished my antibiotics and both the chunky smelly discharge and cut dissapeared.

It was a clean cut where you could sort of see the white flesh underneath. I checked my groin nodes and they were not swollen during the presence of this cut.

I know some people get herpes in the form of papercuts instead of classical blisters. Does this sound consistent with a first time outbreak of herpes?

How common is the presentation of genital herpes as papercuts?

Also people always talk about having antibodies to type 1 from coldsores mitigating or lessening symptoms of a subsequent contraction with type 2.

How does having type 1 orally lessen the symptoms of type 2 genitally? Does it become less noticeable when you already have type 1 orally and a subsequent with type 2 presenting as mild?
I've gotten tested for all other stds after this incident but is a herpes test necessary given this info?

Brief Answer:
Not looks like Genital herpes. Do blood test.

Detailed Answer:
Hello,

Thanks for writing to us. Followings are my comments:

1) According to your information, it is MOSTLY suggestive of Bacterial Vaginosis with blunt trauma from rough sex, for which you already have taken treatment.

2) It DOESN'T look like genital herpes. Typically primary HSV-1 or HSV-2 genital infection occurs in clusters of genital sores with inflamed papules or vesicles on the outer surface of the genitals, resembling cold sores. These usually appear 4 to 7 days after sexual exposure to HSV for the first time.

It is often associated with pain, itching and burning sensation with or without fever, headache, myalgia, lymph node swelling, vaginal discharge etc. You have NONE of these symptoms.

3) Outbreak of Genital herpes may look like paper cut initially instead of classical blisters. However, other clinical symptoms should be associated with it.

4) When a person with NO prior HSV-1 or 2 antibody acquires either virus in the genital tract, a first-episode primary infection results with clinical symptom.

If a person with preexisting HSV-1 antibody acquires HSV-2 genital infection, a first-episode non-primary infection results with or without symptom.

5) Though, it DOESN'T look like genital herpes (as per clinical presentation), I suggest you to undergo one blood test for HSV-2 antibody to clear doubt and confirm the diagnosis. Kindly, consult with your healthcare provider for guidance.

Hope, it helps for your information. Kindly, close the discussion if you don't have any other query.

Wish your good health and take care yourself.

Can a first outbreak of genital herpes resolve in 4-5 days? This healed in 4-5 days. The cut just got smaller and smaller.

In your experience with this virus could the first outbreak present as one simple tiny cut? Is the linear appearance of sores common?

Brief Answer:
Genital herpes doesn't heal too quickly.

Detailed Answer:
Hello,

Thanks for follow up query.

1) It was clean cut wound. That's why it healed up too quickly. Genital herpes outbreak takes time (1 to 6 weeks) to heal and it may require anti-viral medication.

2) Primary outbreak DOESN'T present with simple tiny cut usually. It is commonly associated with classical blisters with other clinical symptoms.

Hope, I have cleared your doubt. Kindly, close the discussion.

All the best and good luck.
