Question: i am having dysntry, i have done my lab test and found that i have some bacterial infection. what should i do get rid off this problm

Hi Sir,

I understand your concern.

Antibiotics for 5-7 days will reduce your infection.

I generally prescribe my patients with oral tablets of Orni-o and Tab.pantoprazole .Please check this with your doctor if he shares my opinion and prescribes the same to you.

I also advise you take ORS sachets and water.

Home based diet along with fruits is advisable.

Get your blood sugars monitored and adjust your Anti diabetic drug doses accordingly by discussing with your doctor.

am 51yr old. i am having fever,body pain, stomach pain,head ache and dysentry.i have done lab test and found that in my urine test there is bacterial infection. i have been told my dr that bcoz of dis infection im having all other prblms. i am diabetic. please help me by prescribing correct medication..antibiotics. i am very tiered bcoz of dis dysentry and terrible pain in stomach.
my blood sugar level is 190 after food. iam taking glim 1mg for diabetics and thyronom 100 for thyroid.
i am allergic to orino can u prescribe some other medication.
hank you for your reply, I am hereby attaching my lab reports please go through it. My blood sugar as of today morning is 190 (fasting) and I take glim 1mg as diabetic medication. I also have thyroid for which I take thyronm100mg medication. Now I am suffering with terrible body pain, fever, stomach pain , diarrhea, headache, nausea, loss of appetite, gastric problem and sometimes shivering is also there. I feel like my whole body is shaking off even my head.

I feel hungry but not able to eat anything, I feel gastric discomfort. I have been taking just soft boiled rice with water and lemon tea. The body temperature sometimes rises to 39 degree celcius, to bring down this I used to have panadol. but the temp goes down for sometime and comes back.

As per your instruction I had 1 dose of Orino -O. Will this alone help me to become better. Can u specify the home diet I have to follow and any substitute for ORS. Is there any vitamin supplements I have to take along with this medicine. Please do reply as soon as possible.

Hello, Madam.

I am extremely apologetic for my delayed response and keep you waiting for the same.

I interpreted your concern.

I have gone through your uploaded lab reports.

Significant abnormalities have been detected, let me list them below.

1.Severe Urinary tract infection(UTI)-Possible Cystis(urinary bladder infection) or Early Pyelonephritis(kidney infection.

2.Uncontrolled blood sugar(FBS 350,190 ,2+) with severe urinary protein loss(4+) -Possible Nephrotic syndrome secondary to Early Diabetic Nephropathy.

3.High TSH levels-Uncontrolled /Inappropriately treated hypothyroid state.

4.Severe Vit D deficiency-Could be secondary to longstanding renal disease or inadequate intake.

5.Elevated Creatinine-Possible Diabetic Nephropathy(DN)/Chronic kidney disease.

6.Significantly positive Antithyroid microsomal antibodies.-Indicate autoimmune thyroid disease possibly Hashimoto's disease.

7.Pertaining to dysentery, It could either be a separate entity or a complication of kidney infection.

Let me offer solutions to each one below in sequence.

1,2,7-I advise you to get an ultrasound(CT scan is more accurate and best investigation) of the kidney's at the earliest along with measurement of GFR(glomerular filtration rate)-This will help to assess the stage of the DN if any.

Antibiotic therapy is needed to cover both infections- UTI and Dysentry.
In case you are uncomfortable with Orni O, it's better to go for alternative antibiotic at the earliest.

Antibiotic therapy is essential in the treatment of acute cystitis or suspected pyelonephritis to prevent progression of the infection. Urine culture and sensitivity testing should always be performed, and empirical therapy should be tailored to the infecting urinary pathogens.

I generally treat my patients initially with Oral tablets of Ciprofloxacin and Panadol for 7 days. This is effective against both UTI and dysentery. Please check with your doctor if he shares my view and if they can be prescribed to you.

This antibiotic therapy should bring a prompt relief to most of your symptoms and should induce a sense of overall well-being.

2-Uncontrolled blood sugar must be corrected with backup insulin since you seem to have low bicarbonate levels which could be an early pointer of body's acid instability .

3,6- High TSH could indicate inadequately treated hypothyroid state.TSH is not corrected even in presence of high dose of T4 -100 mcg..In presence of Positive Antimicrosomal antibodies ,the needof addition of T3 hormone is high to bring the thyroid to normal functional levels and symptomatic improvement. This step is important since in autoimmune thyroid disorders T4 IS NOT CONVERTED TO T3 form due to enzymatic defects.Hence we need to suppplement T3(Liothyronine) externally. With this therapy the overall brain cognition will be increased.

4. Severe Vit D can cause significant body distress in diabetic individuals. It needs urgent correction with Vit D. I generally treat my patients with oral weekly tablets of Tayo K . Please check with your doctor if he shares my view and if can prescribe them to you.

5.Elevated creatinine is a strong indicator of chronic longterm kidney damage due to uncontrolled diabetes.Hence this needs an Urgent GFR measurement to carve further treatment options to halt the disease orogression.

Hence to conclude.

Empirical antibiotic therapy(for both UTI and dysentery) with Ciprofloxacin followed by definitive antibiotic therapy after urine culture and sensitivity reports.

Correction of blood glucose.

Evaluation for DN.

Addition of T3 to thyronorm(t4).
