Question: I have pain and kind of a pinching feel in my lower right back when sitting down.
I had hurt my self doing leg press in the gym
It has been a month since the injury

Detailed Answer:
Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
You seem to have developed Back Sprain following injury in the gym.
Following are the possibilities in your case:
1. Mechanical back pain (Simply Muscular)
2. Prolapsed Intervertebral Disc (PIVD).
The mechanical back pain doesn't radiates to leg while the PIVD pain radiated to leg and is associated with tingling and numbness due to nerve compression.
I would first of all advice you to get a MRI Lumbo sacral spine done to rule out evidence and severity of compression.
If the compression is severe you would require to get a surgical intervention done.
In mild PIVD, Pregabalin helps greatly.
If MRI comes out to be normal you would require to do hot fomentation of the back and apply Diclofenac gel locally. Ultrasonic heat wave physiotherapy also helps.

There is no numbness or tingling but there is a pinch which is better some days that means no pain but just sensation but then starts to feel uncomfortable again . Is there immediate need for MRI ? Is going gym advisable I am taking care of back though in gym

Hi.
I understand that your pain had improved but since it has come again it means there is a possibility of disc slip.
See your doctor and ask him for MRI. Do request him for discount. He will get you MRI done in discounted rates.
It is always better to be sure. So I would recommend you to get MRI done.
You should refrain from gymming till you get back to pain free state.
