Suggest Treatment For Attention Deficit Disorder And Cyclothymia

Question: I am very anxious and find myself unable to handle even minor crises at work. In grade school I was diagnosed as attention deficient but my mother would not consent for pills. I some times lose my thought in the middle of my sentence and find I have switched to another thought right in the middle.

2) How is your energy levels at doing household chores work or at your work place?
3) can you elaborate your compliant of anxiety a bit more? is it persistent for whole day, episodic?
4) how is your sleep? any difficulty at falling asleep problem?
5) body aches?

1, I have normal variety of happiness and sadness but exaggerated in degree and people say I am excessive in emotion what ever it is and I get agitated easily.
2. Energy levels are adequate or excessive. Very hard for me to keep my mind on anything very long. I can't stay still in one place very long.
3. My anxiety level is high but shifts constantly with news, rumor my job might be lost in reorganization, daughter was ill (now OK) that XXXXXXX is winning in Iraq etc.
4. Up until midnight or more every night. Just can't seem to turn off my thoughts
and get ready for bed. I need to be tired to get to sleep.
5. I have no physical impairments other than gluten sensitivity. A nurse friend thinks I have adult attention deficiency disorder or something like that.

I could sum up following inferences from you answers
1) poor concentration at one thing.
2) hyper activity.
3) decreased need for sleep.

Considering the information we have, i could think of few disorders such as
Adult attention deficit hyperactive disorder, bipolar affective disorder, cyclothymia, general anxiety disorder.

Adult attention deficit disorder.
A rare condition. Has an onset in childhood and adolescence, with a chronic course.
Inattention at work is the characteristic symptom of this condition.

Bipolar affective disorder and cyclothymia: These are characterised by mood swings. There will be frequent phases of highs and lows, which could be lasting for couple of weeks.
A period of high can be experienced as persistent elevated mood, or irritable mood, decreased need for sleep, increased pleasure seeking activities, etc.
Following which could be a phase of depression with symptoms exactly opposite to above such as extreme tiredness, low mood, excessive worry, reduced confidence levels,etc.
This illness is usually episodic and there is functional impairment. Cyclothymia being a milder version of bipolar affective disorder.

Lastly general anxiety disorder, which is characterised by persistent vague sense of restlessness, worry, associated with increased heartbeat,etc. there is associated impairment in concentration in due to anxiety.

May i know
1) have some one ever told you that you get angry frequently?
2) do you find yourself making unnecessary purchases and indulging into reckless spending behaviour or deals?
3) Have you taken any medication for this condition in past?

Uptill this far your complaints are strongly suggesting an adult attention deficit disorder.
No confirmation of any clinical condition is possible currently.
However, possibility of mood disorder need to be kept in mind untill a detailed work up is done.

Considering the information we have,
I would suggest
1) Do consult a local psychiatrist soon.
As there is need for mental status examination which warrants presence of psychiatrist in personal. Besides a detailed work up regarding complaints needs to to be evaluated.
A consultation is strongly advisable as there hasn't been any in past.

You can revert back after the opinion of psychiatrist here. Then we can discuss further, depending on the diagnosis, about the management of it.
