Question: I am suffering from anxiety and feeling very weak as well as suffocating and restlessness and also having sleepless nights. I am taking rexipra 10+5 mg and Lonazep 0.5 mg from 24th March, 2014 till date. From few days i am not feeling well. Please advise me for my problem at the earliest.

Thanks for writing to us,

I went through your reports. You are currently being treated with escitalopram 15mg which is an antidepressant and clonazepam which is a benzodiazepine (for sleep trouble) which is tapered off and stopped since it is advisable to not continue it for long period of time. You are suffering from complaints of low mood, loss of interest in activities, anxiety, irritability, reduced energy and several somatic complaints. These are the typical symptoms of major depressive disorder (MDD) for which you are currently being treated. You are also a known case of hypothyroidism.

My advise would be to first get your current thyroid profile done. If deranged, it requires medications to correct it. And this is one of the common causes of depressive symptoms. Secondly, it would be advisable to get a thorough check up done by a physician or internist to rule out any medical causes for these complaints. Some medical disorders can present as generalized fatigue or reduced energy in some individuals. Once this has been done, the complaints can be considered to be due to MDD which is treated with antidepressants. Antidepressants like escitalopram take time to show response, since the dosage has been recently hiked from 10 to 15mg. It takes a minimum of 2 weeks to start showing response. Slowly you will be able to see more improvement eventually.
