Suggest Treatment For Angiokeratoma Of The Scrotum

Question: Hi,
I have several angiokeratomas on my scrotum and I was stupid enough to follow some advice found on the web. This means I have tried to remove some of them using the Wartner verrucas removal kit, which works by means of freezing. I applied the Wartner only for 20 sec on each blister, skin turned white for a few seconds, but now after a few days blisters are still there and I have some very visible red marks. I think I learned the lesson. But now ? What can I do ? Should I just wait and see if this heals ? Can I apply something ? Maybe cortisone ?

I would have preferred a few photographs showing me the latest condition of the skin over there. Can you upload?

I think the skin over there got severely irritated as a result of the topical preparation which you used. The kit was not meant for this area. Scrotal skin is delicate and may have reacted adversely to the topical preparation.

If I was the treating doctor I would have definitely suggested topical application of a combination of topical steroid+antibiotic ointment e.g Fluticasone+mupirocin ointment (Flutibact ointment), twice daily.

Cortisone would be mild. A moderately potent steroid like fluticasone propionate or mometasone furoate would be ideal, preferably in combination with a topical antibiotic

Angiokeratomas are best removed or cauterised with either electrocautery or CO 2 Laser. You should visit a dermatologist for the removal.

I have reviewed the photograph. It is drying up to form a scab. This should peel off in the next 3-5 days. I think it should be fine with the medicines I suggested.

Believe it or not, I just checked in the bathroom and it peeled. Skin looks ok with just a little bit of blood but the angiokeratoma is still there. I guess all the other will do the same. Do you think I still should use some medication ? I asked for cortisone because I have one, Travocort, at home ready to be used.

Hi

That's good. Don't peel them forcibly, let them fall off spontaneously.

You may use just a plain antibacterial ointment e.g either mupirocin 2% ointment or Neomycin Ointment or Neosporin ointment. That would do fine for you.

Angiokeratomas need to be cauterized. You need to go to a dermatologist for that.

Ok, only problem is that I live in Italy and I shoul look for an equivalent product to Neosporin which looks not available. What should I ask for in the pharmacy ? An antibiotic cream ?

Ask for a topical OTC antibiotic cream. 2% Mupirocin is an OTC product in many countries. If you can get it from your local pharmacy, that would be good.

Just ask for a topical antibiotic cream or ointment. The list is: Mupirocin, fusidic acid, Neomycin, polysporin, Betadine etc. You should get one or the other. One of these should be oTC
