Suggest Treatment For Anal Itching

Question: Dear Doctor
My daughter stuffing from itchy around Anus since September
I visited around 7 doctors each 2 to 3 visits
She did stool test fungus culture and it was negative
She took 2 courses of anti parasites and all kind of cream you can imagine
I will attach photo.. sadly along with vaseline it didn't work
My daughter still complaining since September to date
Latest doctors asked me to stop all cortisone ointment and give her anti constipation syrup Duphalac for 3 weeks .. yes she has constipation and her stool was in the shape of balls .. she usually ask to make stool every 2 to 3 days now after the medication she do every day
Till today Docotr I am suffering because doctors cannot see any signs of illness around the Anus .. I am afraid she has anal fracture !?? This will kill me
I am totally lost
Please advise and help

I have gone carefully through your query and reports. It is possible that due to incomplete digestion undigested food matter is left in the intestines and that is degraded by intestinal flora to make the composition of her stool acidic. This is possibly leading to chemical irritation of the anal area leading to the itch. The other possibility is that the body is developing some allergic reaction to some component of stool and that is resulting in the irritation and itchiness. The eczema could be related.

You may rest assured that it is not due to anal fracture.

I would insist you to make her:
* Drink plenty of cleaner water.
* Apply clotrimazole powder perianally
* Take curd or yogurt since they contain plenty of beneficial bacteria.
* You may give some probiotic preparations like Econorm XXXXXXX twice daily for the next few days.
* She should continue Duphalac. She also needs to take plenty of green leafy vegetables and other dietary fiber rich foods.
* Stop cortisone or similar ointments.
You may try experimenting with and altering her diet. Leaving out milk or gluten containing ingredients might help.
* Give her some antiallergic like cetirizine once daily when needed. Montek LC (monteleukast + levocetirizine) is better but is a prescription medicine.
