Suggest Treatment For Adult Onset Asthma And Bronchitis

Question: Hi doctors this is for not me but for a relative. she is 51 years old and it all started back in the winter , she had bronchitis, it got better with prednisone/antibiotics then came back she was placed on meds again- got better but she kept coughing , a lot. they did more testing and found she had "Adult Onset Asthma " During this same time in early spring she started getting hives- chronically she was put on prednisone , and they are now controlled with ONLY allergy meds. Here is my concern, she had a CT done of her chest by the allergist to f/u on a chest xray that showed a nodule . this is the CT results
RESULTS: No discrete lung parenchymal mass. No pleural fluid or
thickening.

Mildly enlarged mediastinal lymph nodes are noted. One of the largest is
noted within anterior right pretracheal region: 1.6 x 1.2 cm (SP 1:15
). A mildly enlarged left hilar lymph node is noted: 1.2 cm. No enlarged
right hilar or axillary lymph nodes.

A round 2.6 cm fluid attenuation lesion is noted within upper right
kidney suggestive of a cyst.

No enlarged lymph nodes within upper abdomen. Spleen not enlarged.

Deformity of posterior aspects of multiple right ribs is noted,
consistent with remote healed fractures. No acute fracture.
IMPRESSION:

1. No discrete lung parenchymal mass. Multiple remote healed posterior
right rib fractures are noted, which may explain finding on chest x-ray.

2. Mild mediastinal and left hilar lymphadenopathy. Findings are
indeterminate for neoplasm. Differential diagnosis includes sarcoidosis
and lymphoma. Consider short-term follow-up CT chest in 3 months.


I am very concerned. what could be the reason? Lymphoma doesn't seem right - she has a NORMAL CBC, she honest to god has no other symptoms . no fatigue, no weight loss, no fever, chills, nightsweats, no pain. her CBC did she something called "High Esopnophils" but that's it
she has also had a normal BMP, Normal SED rate, Normal ANA. She even just had a mamagram that was normal too. and just so you know the nodule ended up being nothing.

I really don't want this to be serious or any type of cancer, her lungs on the cat scan were all clear otherwise

her only symptoms is those hives which are gone now with allergy pills, and this so called "new onset asthma" when she goes outside now with allergies her head gets stuff and her voice gets horse and she has a lot of draining sometimes.

please doctors are you able to ease my mind?

Your relatives enlarged mediastinal lymph node can be due to an infection, allergy or malignancy.
To rule out malignancy i would suggest she does a CT guided biopsy from the enlarged lymph node in her chest.
Since there are no symptoms of fatigue, weight loss, fever, chills, nightsweats or pain and she is only having high Eosinophil count in her blood test the enlarged lymph nodes in her chest are most probably due to allergic reaction and by having allergy pills for few days it should go away.

she has actually been on the allergy pills for probably atleast 2-3 weeks - shes on the allergy pills for the Chronic Hives, she has to take them otherwise the hives would come back it seems, but she does have Seasonal allergies (REALLY BAD) could this be due to the constant sinus drip, seasonal allergies, and chronic bronchitis like symptoms? What about sarcoidosis ? would those things be more likely other than lymphoma due to the fact that she literally has no other symptoms other the hives, and asthma symptoms?

since her CT of her chest was negative (no nodule no masses) other than the lymphnodes that would pretty much rule out Lung Cancer correct? I am sorry I am just worried . and I know she is too

Hi

Sarcoidosis can produce symptoms like swollen lymph nodes ,persistent dry cough and rashes on the shins or ankles which may be warm and tender to the touch and that is why it is important that biopsy from the enlarged lymph node is done to rule it out.

A small sample of tissue (biopsy) is needed to be taken from a part of her body believed to be affected by sarcoidosis that is mediastinal lymph node in her chest in her case to look for the granulomas commonly seen in the condition.
