Question: Hello Doctar,

I am 36 years old Male. I have a question regarding my facial skin.
My skin on the face has always been prone to acnes that secrete white sebum on being plucked. These are more prone on the chicks, foreheads, sides of the nose etc.
In the past i have tried Salicylic acid OR Benzoyl peroxide based products. But the problem is not eliminated.
What do you recommend as a solution to treat this problem from the root?
(a) Are there any laser treatments that can be tried safely?
(b) Are there any other treatment/suggestions you have?

Regards,
doctor

Brief Answer:
I would suggest treatment with isotretinoin

Detailed Answer:
Hello. Thank you for writing to us.

I am dr.kakkar(dermatologist). I have gone through your query and i have noted your concern.

Oily skin is prone to acne. Hence, controlling or reducing oil would help control acne.
Though benzoyl peroxide and salicylic acid help dry excess oil thereby benefitting acne, they don't reduce overall sebaceous gland activity and oil secretion. The action of these medications is more superficial.
The most effective medication to reduce sebaceous oil secretion is Oral Isotretinoin. Isotretinoin helps by reducing activity and size of oil glands by 90% within a month of initiating. Topical antiacne medications i.e benzoyl peroxide gel or clindamycin gel can also be continued beside isotretinoin.
Isotretinoin is a prescription medication and should be used under supervision of a dermatologist. Lab parameters like liver function test and serum lipid levels should also be monitored while on isotretinoin.
