Question: Doctor,

I'm a mess. keep crying. tomorrow I am.seeing my dermatologist but I found a waxy sore nodule, little blood cut on arm. do I have basel or squamous cell. I am.37 female with history of moderately aytipical moles. I'm scared please tell me your views.photo attached

I have gone through your query and I have also reviewed the Images.

This nodule could just be an acne lesion on your arm. You said that it is sore. A BCC is never sore.

However, I would like to gather more information from you in order to be able to guide you better.

-How long since you have noticed the nodule?

-I can see that in the background of this nodule you have a somewhat scaly rash? Is this rash itchy? the rash looks like Fungal infection to me and probably you scratched it and also the nodule.

-Do you have this rash elsewhere?

-Are you using any cream for the rash?

Hi, I have bad excema. which is what you see. I am.using cortisone cream for this. The nodule seems like it has a cut around it. the nodule is firm when I touch it is hurts alot. I'm sending you a clearer image. I am concerned about squamous or basel and am crying alot.

I have reviewed the Images.

A basal cell carcinoma (BCC) or a squamous cell carcinoma (SCC) is never sore/tender or hurts.
I think this is most likely an acne nodule probably secondary to the steroid cream that you may be using for eczema and it is sore because it has probably got some inflammation.
Probably you scratched it because it is against a background of eczema, which must be itchy, and thus may have caused a cut around it.
I suggest an oral antibiotic e.g Azithromycin Or Lymecycline. Topical you may use an antibacterial e.g nadifloxacin cream or gel Or topical benzoyl peroxide.

I wanted to know since how long have you noticed it?

Hi.

I would treat it as an inflammatory acne nodule with Oral antibiotics and Topical's that I mentioned.
Since it it firm and tender/sore, it rules out a BCC or an SCC.

Has it increased in size recently?

it is firm but it's only sore where the cut is at the side. it's waxy which is why it's making me think cancer etc. I think it got a little bigger which is what made me think what's this. I haven't used the cortisone cream in a while either. So worried.

Can understand your anxiety on this one.

A BCC nodule is a very slow growing lesion and it may ulcerate in the center rather than at the sides.
The surface usually show telangiectasia/minute blood vessels across it. It would require a close inspection, though.
Moreover, the most common site for nodular BCC is face.
