Suggest Treatment For Acne And Pigmentation

Question: Hi am using comedolytic 0.025% cream on my face from past 7 days .. for first few days .i felt burning sensation and some peeling also happened , But now my face becomes very dark and some dark patches are seen in my face. I am getting some burning sensation on my skin and it is becoming very dry. am using sunscreen lotion also please suggest me on this .

I am Dr. Kakkar. I have gone through your query. I have also looked at the Images.

You have 2 main concerns: Acne and Pigmentation(Melanosis).

The cream that you have been using for acne (comedolytic cream) contains tretinoin 0.025% which is an excellent comedolytic/antiacne. It may cause some initial peeling and dryness, irritation, however, these side effects gradually lessen with time as the skin gets used to the cream.

Regarding pigmentation, I want to gather more information from you in order to be able to guide you better:

-How long has the pigmentation/patches been there? Were they present before you started using comedolytic cream? I can see a band of pigmentation on the temples and across forehead and diffuse pigmentation on the cheeks as well. It seems that it is long standing.
-Do you use any other cream on face apart from sunscreen e.g an OTC fairness cream etc?

My skin appears some dark near my fore head and around nose. I am using biotique sunscreen lotion. You can see in photos , after started using comedolytic cream i say dark patches near fore head area and cheeks.

befor using comedolytic cream only around nose area some dardness is there.

Also my face become very dark after using Clyndamycen phospate gel and Comedolytic cream,

shall i continue using Comedolytic cream or not let me ..

Hi.

Comedolytic cream contains tretinoin which is a good antiacne molecule but it can cause irritation and sometimes patches on face. The darkness that I see on the temples and forehead is unlikely to be due to tretinoin. Anyways I would suggest you to discontinue comedolytic cream(tretinoin) because may be it is too strong for your skin.

Clindamycin does not cause dark patches.

Give your skin a break for 3-5 days.

Thereafter start using Deriva CMS gel at night for 2 hours on acne affected area and wash it off with a gentle cleanser after 2 hours. You may use a skin lightening cream e.g Kojic cream twice daily ( morning and evening). In the morning use kojic cream 1 hour before using sunscreen and in the evening use Kojic cream after 2 hours application of Deriva-CMS gel and leave it overnight.

Besides, You may also start oral retinoids (Isotretinoin) e.g Cap Sotret 20 mg once daily at night after meals, for acne.

Stop using soaps on face. Rather use a gentle cleanser for face wash e.g Cetaphil cleansing lotion twice daily.

Few moths back i tried using Deriva -CMS gtel and Kojic Cream . Even after using that for few months i didn't get any Results. As of now i stopped using the comedolytic cream from past 2 days. Is there any other Creams which suits for my skin.

Hi.

Azaleic acid 20%(Aziderm 20% gel) twice daily, rub it in gently. Azaleic acid works for both acne as well as hyperpigmentation. Use it twice daily.

You may also start oral retinoids (Isotretinoin) e.g Cap Sotret 20 mg once daily at night after meals, for acne.

Use a gentle cleanser e.g Cetaphil cleansing lotion, for face wash.

HI,

can you please explain what is the process and how i have to applythe aziderm 20% cream.

i a having dark patches near forehead and cheaks , i an directly apply for that areas or i have to apply for entir face.

My face become very wore due to this dark patches . As my skin is very sensitive , this cream wont create any further problems right.

Kindly suggest on this.

Hi.

Azaleic acid works for both acne as well as pigmentation. Therefore you need to apply it to your entire face.

Pigmentation would take time to improve.

Since your face is already irritated, that is why in my first reply, I asked you to give your skin a break from all medicines for 3-5 days before starting Aziderm 20% gel.

One more thing I want to add is that you may stop using sunscreen for now because sunscreens can also cause acne in some individuals. Avoid going out in sun specially between 10 AM to 3 PM and if you anyways stay indoors during most part of the day there is not much requirement for a sunscreen.

Use a gentle cleanser for face wash e.g cetaphil cleansing lotion, twice daily.

Also begin with Sotret 20 mg capsules.
