Question: Hello,
My name is XXXX. I am trying to reach out to you regarding the condition which I am in right now and this is continuing for last 8 months. I feel very weak, lethargic, if I any work I feel all my energy is drained. I need to lie down on my bed until I get my energy back also I feel suffering for breath. I have wheezing problem. I have visited several doctors till this time but not able to get right treatment.
I live in XXXX and have visited many Primary care as well as specialists to get an answer on what is happening to me. My primary care suggested me to take Pantophrazole 40 mg in the year 2016 as I was having GERD problem. I have been taking it daily since then, if I miss then my Acid refulx gets worse and I feel burning sensation. then my doctor suggested me to skip 2 days and then take pantophrazole. I am having PCOS and also pre-diabetic so was advised to take Metformin 500 mg. i was taking this since 2016 as well. My PCP advised me to stop Metformin on Feb 2017 and since then I started missing on my periods and started having Pelvic pain. I had to go to emergency room due to this as I felt like i am going to die. 8 months back i was having nerve pull in my whole body which gave me sleepless nights so I visited a Neurologist and was told to have Gabapentin which I took for sometime. I was told to do a Nerve conductor test which I could not perform due to weakness. I have mentioned this to the Neurologist, but was told if I do not do the Nerve conduction test that is fine, but take the Gabapentin if needed. Again my PCP advised me to stop taking Gabapentin so I stopped taking since Feb 2018.

Due to this shortness of breath I visited Pulmonologidt at Mayo clinic and was told to take Asmanex as I was told I have mild ashtma but after taking Asmanex this ashthma problem so I had to switch my Pulmonologist and was told to stop taking Asmanex.

After all this I had to switch my PCP as I not satisfied with his treatment. My recent Primary care physican has advised to stay on organic food and stay away from Pantophrazole as that can cause lot of problems. I had colonoscopy in 2016 so he mentioned having Pantophrazole for long time caused lot of damage. he asked to take Apple cider vinegar before lunch and not take pantophrazole. But I had to take it every 2-3 days as I still face acid problem. I was told to have Chicken bone broth from Kettle and fire brand along with Vitamin B12 and have a walk for 20 mins in the morning. I was told to consume fresh but not sure where we can get fresh fish as all are farm fresh not atlantic.

I really need advice so I get recover fast as I cannot handle this anymore. My entireright side is heavy and keeps paining. I had HIDA scan earlier in 2017, my Gall bladder ejection fraction is 46% so it feels heavy. I just want to get cured. Moment I wake up my body keeps giving me signals that you are not well. I am completely helpless as I cant find a good treatment in spite of changing doctors. Sometimes I just can;t believe I am in US.

Please help me.

Welcome and thanks for your query.

I understand your concern, having suffered from multiple problems for the past 8 months and not finding lasting relief from the different treatments.

One of the main problems is due to acid-peptic disease. The usual reasons for this are curry, worry, hurry, and Pylori. You have to take things easy and be relaxed and manage any stress through healthy wholesome food, an exercise of 30 minutes or more, practice stress management like Dr. Jacobson's relaxation techniques or yoga. You could get a breath test or stool test for the H Pylori. If present, treating it would give you more lasting relief.

The presence of PCOD and GERD are suggestive that you may be overweight. Losing weight would help to get better regarding both problems. Metformin can produce gastrointestinal side effects and this is less with extended-release preparation like Glucophage XR. Avoiding fatty foods would help your right sided pain if it is gallbladder in origin.

Hello Doctor,

Thank you for replying to me. I understand what you are saying. But right now I am low in energy. I feel so tired, exhausted, fatigue all the time. I can't do any household chores. If I sit in the car I feel exhausted, tired and have shortness of breath. I love Yoga but not sure how much I can do. I used to do Yoga earlier when I was fine.

My current PCP has suggested me to stop pantophrazole and use Apple cider vinegar, consume Chicken bone broth, organic food, rainbow veg, Vitamin B12, enough sunlight to get Vitamin D. Will this help to get back my energy.

Welcome back.
If you have abdominal and retrosternal burning, you need a PPI . If Pantoprazole does not suit you, you can use esmoprazole ( Nexium).
Your lack of energy can be due to hypothyroidism or masked depression.
If you have not had Thyroid function tests done, it may be good to have the same done. If you have not had your blood checked for Vitamin D level in the past 3 months, the same may be done. If they are normal, it may be worth getting a psychiatric assessment, if you do not have any reservations about the same. An SSRI may help you get better. Exposing yourself to sun in a safe way is good. Vinegar may worsen your symptoms of abdominal and chest burning. Others are ok.
