Question: About 2 months ago, I scratched an itch in the crack of my buttock. I noticed quite a bit of dead skin came off when I did and it hurt quite badly. Since the, I've had a very painful open area in the upper portion of my buttock crease from the top to about half way down,( it doesn't go all the way to the anus.) I've tried numerous creams and ointments with no success. Can you please tell me what I can use to heal this for good? It's incredibly painful when I sit and when I shower. It has not bled, it's just red and cracked open slightly.

Thanks for consulting me at virtual clinic. Your trust in HCM means a lot to me as a Physician.
I have completely reviewed your health query, and can well realize all those concerns. Being your physician, I will take care all of them today and will give extend my expertise to solve the problems.

You have got infection on the skin due to scratching and now you need dual antibiotic therapy (oral and topical) as this is very pathogen rich moist area.

If I was your treating Physician I would have prescribed you with:
Cephalexin oral if not allergic to (it is necessary to get rid of the infection).
Neosporin ointment topical.
ProCort ointment topical (it will soothe you and will relieve the itch and inflammation too).
Ibuprofen for pain.

Take a shower with Oatmeal milk and honey body wash. No scented soaps. Keep the wound clean by using an antiseptic.

Take plenty of water and fluids and keep a balanced healthy diet.

Hope I answered all of your queries. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you with my expert opinion.
