Question: Dear Dr,

What medical treatment (what to use, for how long) would you recommend for a patient suffering from excess hair growth, irregular menses possibly caused by PCO? Patient is already on a diet and has lost a lot of weight but is still experiencing problems.

Blood tests show the following:

S-Prolactin 166 71-567 mIE/L
S-Progesterone 3,6 follicle phase 17; post menopause <3,0

I have gone through the details provided. The lower levels of SHBG are indicative of insulin resistance and PCOD.
PCOD is an endocrine condition in which there is high LH/FSH ratio,increased androgen production and high oestrone levels.It is usually associated with abnormal hair growth , obesity , menstrual disturbances and infertility.
Treatment includes lifestyle modification in which weight reduction and diet plays an important role.
Metformin drug is widely used for the treatment of PCOD but should not be taken without doctor's guidance.
Low dose oral contarceptives and progesterone are also given for menstrual irregularities.
Ovulation induction drugs are given for infertility.
Some doctor's prefer laparoscopic ovarian drilling but chances of adhesions are there.
For hirsuitism laser , waxing or chemical depilation can be done.
All these treatments are to be continued till the irregularity of periods is corrected. The duration of treatment is not fixed. It depends on the response to treatment.
To conclude PCOD is a very common metabolic disorder these days and is due to western life style and inactivity , can be controlled with proper food , exercise , medical and surgical treatment under doctor's guidance.

What natural options are there to decrease the androgen levels?

The natural treatment of PCOS and raised androgen levels involves loosing the extra body weight with the help of a healthier diet, getting more exercise, and then supplementing with vitamins and herbs.
Balance low GI carbohydrates with adequate protein.
Include phytoestrogens in your diet.
Buy organic foods whenever possible.
Reduce your intake of saturated fats.
Limit or avoid dairy.
Drink two litres of filtered water daily.
Avoid additives, preservatives and food chemicals such as artificial sweeteners.
Avoid caffeine and alcohol.
Avoid sugar on its own and hidden in foods.
Include essential fatty acids in your diet.
Herbs like chaste berry, milk thistle and dandelion root are also helpful along with acupuncture.

Could you also please let me know what natural options there are to raise the estrogen levels? Is there any over-the-counter medicine or estrogen cream that you could recommend?

For PCOD an over the counter topical estrogen preparation will not help. The topical preparations only show a local effect. You can take natural phytoestrogens. These can help you.

Dear Doctor, thank you for the answer.

What do you think of soy-based phytoestrogens? My understanding is that there is no consensus regarding the positive or negative effects of soy for PCOS patients.

Would it be safe to take soy based phytoestrogens given the results from the blood tests above?

Hello.
Soy based phytoestrogens can be helpful in the above situation. There is no harm in taking the soy based products in PCOD.
However it will take at least 4-6 weeks for such a treatment to show an effect.
