Suggest Treatment For A Painful Hemorrhoid

Question: Hi , I have a painful haemorrhoid since last three days .No bleeding .Had undergone stapling technique surgery 14 years ago for piles . Since 2 days used Warm water with salt Sitz baths . Using Onabet B cream , and taking one Combiflam a day , but not much relief .Also have used Prepration H cream but no relief Advice thanks XXXX

Hello! Thanks for putting your query in HCM. I am a Gastroenterologist.

Hemorrhoids become painful only when they are thrombosed which is an emergency, may require surgery and you should be admitted and in the presence of anal fissure.

If it is an anal fissure then:
1.Take lots of fruits and vegetables to increase fibre to make stool bulky
2. Add ointment containing nitroglycerine which will help in relaxation of anal sphincter which will relieve pain and fasten the healing. To be used before passing stool
3. Lox ointment for relieving should be used 4 to 5 times per day
4. Take a laxative laso to make stool soft. Among allopathic medicines you can take Lactifibre which is the combination of lactitol and Isabghola, which are quite safe even for long term use. May also add polyethyleneglycol which comes in XXXXXXX or syrup form.
5. Continue with sitz bath

So visit a Gastroenterologist or a Surgeon for evaluation and manage accordingly.

So should i stop using Onabet B , betnovate and stop consuming combiflam ?

Onabet ointmet contain antifungal agent and steroid. And betnovate ointment contain steroid anribacterial and antifungal agent. So you may use betnovate ointment only. Combiflam tab do help in reduction so you can use it twice or thrice daily .

Hi , wanted to enquire as to how to manage or alternate Lox 2% jelly and Betnovate GM ointment over a day .Can they be used together .

The heamhrroid size hasreduced and pain also less . But just an hour ago after a shower bleeding/ spotting is there . Advice

For bleeding hemorrhoids the combination of tanlet calcium dobesilate and tablet diosmin are to be used both thrice in aday for 10 to 14 days. Keep stool soft by taking lots of fibres and fluid and laxative
