Posted on Wed, 19 Aug 2015
Question: I have a UTI.....cloudy and foul smelling urine, extreme lower back pain. I was nauseous and vomiting Friday and Saturday but that has dissipated. I also have trembling and loss of appetite. I am 57 years old and have experienced several UTIs over the last 8 months, having never had one before, always treated with antibiotic This is worse than the others. I intend to see my doctor tomorrow but is there anything I can do in the meantime to ease the discomfort.

Read detail answer below.

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that you have low back pain and burning urination ,nausea and vomiting since 3 days .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have increased frequency for urination ,poor stream or urgency for urination ?
2) Are you diabetic?.

Repeated episodes of UTI suggests that it could be likely secondary to Enlarged prostate which is common at this age .

Please get your routine urine test and urine culture done to find out the organisms causing this infection and antibiotics to which they are sensitive to.

Please consult qualified Urologist for clinical and digital rectal examination and get following basic
tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.

Please take broad spectrum antibiotics like Cefixine along with urinary antiseptic like Nitrofurantoin twice daily and urine alkalizer thrice daily .Later on switch on to appropriate antibiotics as per culture report.

Ensure to drink more water .To keep your urine dilute This will help to control dysuria.

Further treatment will depend upon result of these tests and final diagnosis.
.
Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.

I am a 57 year old female so I don't believe I have a prostrate. Yes, frequency of urination. I am already being treated with Vesicare for that. I believe I answered my own question. Thank you for your time.

Brief Answer:
Sorry for wrong interpretation and wrong posting.

Detailed Answer:
Hello
I am extremely sorry for wrong interpretation and wrong posting .

All the symptoms that you have especially repeated episodes of UTI over last 8 months are mostly due to what is called as Post Menopausal Urethral Stenosis.

After the menopause or during peri menopausal age there is generalized atrophy of genital organs including urethra. This results in to narrowing of urethral passage .This narrowed urethra causes incomplete emptying of bladder during micturation resulting in to significant amount of urine to remain in bladder (Post Void residual Urine).
This post void residual urine is the source of getting repeated episodes of UTI .

Please consult qualified Urologist for clinical examination and get following basic
tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis with specific mention of post void urine volume .
3) Endoscopic examination of bladder (Cystoscopy)

If he finds Urethral passage to be narrowed he may do Urethral Calibration simultaneously to widen the urethral passage .This will help to empty bladder completely and there by prevent getting UTI repeatedly.

Thanks and Regards with apology for my grave mistake .
