Brief Answer:
She appears to have severe OCD.

Detailed Answer:
Hello,
Thanks for your query.
I appreciate your efforts for medical help for your mother.

Important aspects of your query are:
•     54 year old female
•     Excessive cleaning and excessive-irrational fear of contamination.
•     She is not accepting treatment.

Her repetitive fears related to infection/contamination appear to obsessional thought and repetitive washing appear to compulsive behaviour to decrease anxiety related to obsession. Obsession and compulsion are seen in obsessive compulsive disorder (OCD).

OCD is a type of anxiety disorder. If these disorders are not treated early they can lead to depression because of distress and dysfunction.

Common symptoms of anxiety disorders : anxiety, nervousness, restlessness, mild irritability, excessive worry regarding own wellbeing, tremor, muscle spasm, heaviness in body, easy fatigability, palpitation, sleep difficulty and dry mouth. Many of your complaints may match with this.

She needs evaluation by psychiatrist to confirm diagnosis and start treatment accordingly. In such cases treatment can be given only after in-person evaluation of patient.

In drug treatment most commonly SSRI (selective serotonin reuptake inhibitors are used. SSRI takes around 3-4 weeks to show its effect and cognitive behaviour therapy (counseling treatment for OCD) also takes weeks to show its effect. These medicines are prescription medicines.

So, it is better to persuade or force your mother to be checked by nearby psychiatrist (best option). If she does not go to doctor, then search some nearby psychiatrist who can provide home visit (costly option). In very severe cases forceful treatment may be required after admission of patient in hospital (like if condition is life threatening and Patient do not accept treatment).

I hope this information has been both informative and helpful for you. In case of any doubt, I will be available for follow ups.
Wishing her good health.
Regards
